The Effectiveness of the Transitional Care Program Among People Awaiting Coronary Artery Bypass Graft Surgery: A Randomized Control Trial

Abstract: Coronary artery disease (CAD) is a significant health problem in Thailand and worldwide (World Health Organization, 2018). When the conditions cannot be controlled by optimal medication therapy, further treatment is based on coronary artery bypass graft (CABG) surgery (Neumann et al., 2019). The international guidelines recommend that the people with stable CAD who are likely to benefit from undergoing CABG for revascularization should have CABG performed within 6 weeks (Neumann et al., 2019). In Thailand, mo… Show more

“…This systematic review aimed to analyse the effects of nursing interventions based on continuity of care in patients with coronary artery disease. After hospital discharge, providing education, support, and continuous home monitoring to patients with coronary artery disease is necessary [ 4 , 45 ]. Therefore, patient education is a fundamental component in the continuity of care, being a nursing role [ 14 , 46 ].…”

“…After hospital discharge, these patients usually showed psychological disorders, changes in family dynamics, and even professional problems; therefore, home monitoring and social support allow them to improve self-efficacy [ 51 ]. In addition, as other authors indicate, providing information on the management of cardiovascular symptoms, reporting complications associated with surgical intervention and wound care is essential [ 45 , 49 , 52 , 53 ].…”

However, four studies did not mention the frequency of intervention delivery.…”

“…The transitional care program intervention emphasizes providing information regarding the initial management of heart symptoms, lifestyle modification, and the use of medication packaged in a heart book/booklet. 28 This intervention was led by a cardiovascular specialist nurse and was carried out during 2 phases, namely during discharge planning and after discharge from the hospital. Health education was provided for approximately 20 minutes before discharge, and follow-up was carried out by telephone 48 hours after discharge from the hospital.…”

“…A meta-analysis confirmed that TC significantly declines hospital readmission and mortality rate in COPD subjects [ 12 ]. For patients awaiting coronary artery bypass graft (CABG), TC can reduce anxiety, depression, and hospital times while enhancing cardiac self-efficacy and satisfaction with care [ 13 ].…”
