The effectiveness of lifestyle interventions to reduce side effects of androgen deprivation therapy for men with prostate cancer: a systematic review

Abstract: Purpose The aim of this review is to systematically review randomized controlled trials on lifestyle interventions on PCa patients undergoing androgen deprivation therapy. Methods A literature search was conducted using the electronic databases Medline and PubMed. To be eligible, studies had to be randomized controlled trials (RCTs) that focused on side effects of ADT and lifestyle interventions to reduce side effects for men undergoing ADT with PCa. Lifestyle interventions were defined as interventions that i… Show more

“…To improve the QOL of patients receiving ADT, it is important to conduct routine screening to identify fatigue in patients. 21 In our study, the dyspnea scale score increased by 13 points over 12 months compared with the pretreatment values. The mechanism underlying dyspnea in patients with cancer is not well understood due to the heterogeneous origins of shortness of breath.…”

“…Diepernick et al (37) in a follow-on study found that that pelvic floor muscle strength of men in their intervention study diminished over the 3 years post-intervention but that men still had better LUTS than men in their control group. The evidence for lifestyle interventions for reducing PCa treatment adverse effects is growing [38,39]. Furthermore, whilst a recent systematic review [40] has classified the important components of benign LUTS self-management, the active components or behavioural interventions that contribute to these are far from clear.…”

“…It should be noted that these interventions may have potential adverse effects such as increased anxiety, so the patient should still be monitored appropriately as with other interventions 24 . Vigorous monitored exercise is particularly effective in treating the side effects of ADT in men with metastatic disease 25 . It is also important to consider the experience of partners, who can suffer profound depression and anxiety because of a PCa diagnosis in a loved one and may need assistance in identifying depression or anxiety and accessing appropriate care.…”

It may be appropriate for individual patients and couples to be referred for psychosexual therapy and sexual rehabilitation.…”
