The effect of telmisartan and ramipril on early morning blood pressure surge: a pooled analysis of two randomized clinical trials

Abstract: Telmisartan significantly reduced the early morning systolic blood pressure surge compared with ramipril. A reduction in this surge may help to reduce cardiovascular events in the morning period.

“…The fall in DBP has been noted even at the first follow-up at two weeks. Our observations in respect to DBP are in line with the reports of previous studies 24- 26 . Thus, it can be stated that ramipril causes fall in SBP as well as DBP and that the fall in BP may be noted as early as two weeks of therapy.…”

“…Amlodipine and telmisartan have long half-lives and, therefore, long duration of action. Indeed, telmisartan provides longlasting and consistent BP control over a 24-h period compared with other renin-angiotensin system inhibitors [9,[27][28][29], and this is the case also for amlodipine when compared with other calcium channel blockers [30,31]. Visitto-visit variability in SBP and maximum SBP are strong predictors of stroke, and in patients with treated hypertension, increased 24-h residual variability in SBP is associated with a high risk of vascular events [32].…”

“…The early-morning surge is one of these BP variability components, and has been linked with target organ damage and stroke [9][10][11]. Night-time BP patterns are also of importance as a higher nocturnal BP and an increased night-to-day SBP ratio, indicating a reduced nocturnal BP fall, are both independent predictors of cardiovascular events [2,[12][13][14].…”

“…Inne analizy przeprowadzone w podobnej podgrupie pacjentów pokazały, że stosowanie telmisartanu jest korzystniejsze w odniesieniu do redukcji porannych wzrostów wartości ciśnienia tętniczego (tzw. morning surge) [28].…”
