The effect of semi-quantitative T1-perfusion parameters for the differentiation between pediatric medulloblastoma and ependymoma

Abstract: Background: The differentiation between medulloblastomas and ependymomas plays an important role in treatment planning and prognosis for children. This study aims to investigate the role of T1-perfusion parameters during the differentiation between medulloblastomas and ependymomas in children. The institutional review board approved this prospective study. The brain magnetic resonance imaging (MRI) protocol, including axial T1perfusion, was assessed in 26 patients, divided into a medulloblastoma group (group 1… Show more

“…Malignant tumors display increased angiogenesis compared with lower-grade tumors, due to the upregulation of proangiogenic factors (VEGF and bFGF) and the downregulation of inhibitors (interferon and angiostatin). Increased angiogenesis is the primary explanation for why perfusion differs between medulloblastoma and pilocytic astrocytoma ( 4 6 , 8 , 18 26 ).…”

“…Any increases in the cellular density, odd substances, or heavy particles within those spaces will result in a reduction of the diffusion coefficient [ 8 , 9 , 10 , 11 , 12 , 16 , 19 , 20 ]. Medulloblastoma is a malignant brain tumor, characterized by high cell numbers and density [ 8 , 9 , 10 , 11 , 12 , 13 , 14 , 15 , 16 , 20 , 21 , 22 , 23 ]. The reduction in free water movement caused by narrow inter- and intracellular spaces will result in reduced signal production.…”

“…Therefore, the speed of apparent diffusion in a solid, dense tumor like ependymomas, would be small. 9,10,12,19,21 Inversely, gliomas in the brain stem are less condensed. The intercellular space is usually larger, such that the protons of hydrogen are not greatly restrained.…”
