Striae distensae are a frequent skin condition associated with pregnancy, weight change or lack of skin elasticity. The aim of this research was to obtain a topical product containing herbal active ingredients with documented antioxidant and anti-inflammatory activity (Punica granatum seed oil and Croton lechleri resin extract) and demonstrate its positive effect on prevention and treatment of striae distensae. First, the cream base formulation was optimized through experimental design. Secondly, the cream containing the two active ingredients was investigated in an interventional nonrandomized clinical trial. The clinical outcome was assessed through biophysical parameters and ultrasonographic evaluation. The state of the skin was evaluated by biophysical measurements and ultrasonography at the beginning of the study and after 3 and 6 weeks. The experimental design was successfully used to set the best ranges for the technological and formulation factors to obtain a cosmetic formulation with optimal characteristics. The study of clinical efficacy on the optimal formulation revealed an increase in the dermis thickness, hydration and elasticity values in both groups after 6 weeks of cream application. The new oil-in-water cream containing P. granatum seed oil and C. lechleri resin extract can be helpful in the prevention or improving of skin changes associated with striae.

Striae gravidarum (SG), or stretch marks developing during pregnancy, affect up to 90% of women. While not medically dangerous, SG can be disfiguring, causing emotional and psychological distress. However, studies specifically addressing the prevention of SG, especially during pregnancy, are sparse. Furthermore, the molecular pathogenesis of SG is unclear and may differ from that of striae from other causes. Considering these factors, we review topical modalities that have been used specifically for preventing SG during pregnancy. We identify two major strategies (end points) addressed by these modalities, namely (i) preventing the de novo development of SG and (ii) reducing the severity of SG that have recently developed. We also identify risk factors for the development of SG and suggest that pregnant women with these risk factors are an appropriate target population for prevention. In reviewing the literature, we find that there is limited evidence that centella, and possibly massage with bitter almond oil, may prevent SG and/or reduce their severity. There is weak evidence that hyaluronic acid prevents SG. Tretinoin holds promise for reducing the severity of new-onset SG, but its use is limited by its pregnancy category. Finally, cocoa butter and olive oil are not effective for preventing SG or reducing the severity of lesions. We conclude that reliable methods for preventing SG are scarce. Furthermore, available topical modalities generally lack strong evidence from rigorous, well-designed, randomized controlled trials with ample numbers of subjects. Thus, further research is necessary to elucidate SG pathogenesis, which may lead to effective prevention modalities.

S tretch marks are manifestations of epidermal atrophy and result from tissue damage due to excessive tension [1, 2]. Stretch marks are one of the most common skin lesions for both women and men. The most frequent places of their locations are the abdomen, buttocks, thighs, chest, sleep, armpits and groin. Striae are divided according to the type and etiology on atrophic, stretch marks be-belt, linear stretch marks, red stretch marks, white striae, striae black and dark blue [3, 4]. Blue and black stretch marks are less common than other clinical types of stretch marks, especially among patients with dark skin phototypes due to the increasing saturation of melanin and in the foci [5, 6]. Due to its location in a particular anatomical region for stretch marks is characterized by the following pattern. Pragnacy stretch marks are predominantly localised in the abdominal area and on the breast; the adolescent stretch marks are localised on the thighs and in the lumbosacral region; nonpregnant young women get stretch marks mainly in the chest area and hips [5, 7, 8]. Most often striae are perpendicular to the lines of Langer, in directions with a minimum capacity of the skin to stretching [1, 2]. Striae represent linear atrophic scars, able despite the small area and, most often, the localization at closed clothing parts of the body, cause significant psychological difficulties and lower quality of life due to aesthetic concerns, and because of such subjective symptoms as itching [5, 9, 10, 11].
