IntroductionPeople with cirrhosis of the liver are predisposed to developing oral lesions. The occurrence and type of lesion depend on the degree of liver function impairment and its type, and on the severity and duration of systemic diseases. In children, the age at which the early symptoms of liver disease are experienced is also of great importance.AimTo assess the prevalence of oral pathological lesions in children and adolescents with cirrhosis of the liver, and their correlation with the degree of liver function impairment.Material and methodsClinical and laboratory results of liver function tests (Model of End-Stage Liver Disease/Score of Paediatric End-Stage Liver Disease, Child-Pugh score) were assessed in 35 patients with cirrhosis of the liver. The average age of the patients was 10.7 ±4.74 years. All patients also had their oral cavities examined (mucosa, gingiva – GI, hygiene – PLI, teeth – dmft/dmfs and DMFt/DMFs, DDE Index and Candida spp. presence) and this was then correlated to the degree of liver function impairment.ResultsAccording to the Child-Pugh scale, 16 patients were class A and 19 were class B/C. Jaundice during the first 3 years of life occurred in 9 patients. Mucosal lesions were found in 26 out of 35 patients (74%), including 10 out of 16 (63%) in Child-Pugh group A, and 16 out of 19 (84%) in group B/C (NS – non significant). Oral candidiasis occurred more often in class B/C than in class A (47.4% vs. 12.5%; p < 0.05). The GI index (Gingival Index) and PLI index (Dental Plaque Index) did not differ between the groups (A vs. B/C) but correlated in the whole group (R = 0.58) as well as in subgroups A (R = 0.65) and B/C (R = 0.59). Dmft/dmfs and DMFt/DMFs indexes did not differ between groups A and B/C, and neither did the DMFt/DMFs in patients with/without enamel defects.ConclusionsOral mucosal lesions are commonly found in children with cirrhosis of the liver. Advanced liver disease promotes oral candidiasis. Severity of gingivitis correlates with the presence of dental plaque.

Vitamins A, D, and microRNAs contribute to T cell differentiation into TH2 phenotypes. We investigated the molecular mechanisms and effects of vitamin A and D on the expression of GATA3 and miR-27-3p isoforms in experimental autoimmune encephalomyelitis (EAE) animal model of multiple sclerosis. EAE was induced in C57BL/6 mice by immunization with myelin oligodendrocyte glycoprotein, mixed with Complete Freund's Adjuvant, together with injection of pertussis toxin. Treatments began one day before immunization with (200 μg and 100 ng of vitamin A and vitamin D per mouse, respectively, and vitamin A+D (100 μg+50 ng) per mouse. Expression levels of GATA3 and miR‑27‑3p isoforms were measured in the CNS and splenocytes by real-time RT-PCR. The expression level of GATA3 in the mice spinal cords and splenocytes was increased in the vitamin A and A+D-treated EAE mice at 24 h and 48 h after restimulation by 10 µg and 40 µg of myelin oligodendrocyte glycoprotein. Vitamins A and D and their combination upregulated the miR-27-3p isoforms compared with EAE mice with no treatments. We also demonstrated that miR-273p isoform expression was altered in splenocytes of vitamin-treated EAE mice. The results showed a positive correlation between splenocyte GATA3 levels and miR-27-3p isoform expression. The protective impacts of vitamins A and D in EAE mice may be mediated by the upregulation of GATA3. However, it is not specified whether suppression of GATA3-targeting miRNAs of the miR-27-3p family is involved in this effect. These results do not rule out the possibility that miR-27-3p isoforms might have beneficial effects by targeting other transcripts, such as GluA2 and NR2B.

IntroductionProper nourishment is one of the basic elements in the treatment of patients with both acute and chronic pancreatitis. Following acute pancreatitis, the essential element of nutritional treatment is dietary education. The dietary approach is used not only for treatment, but also for prevention of reoccurrence of the condition.AimTo evaluate nutritional knowledge of patients with acute and chronic pancreatitis.Material and methodsThe selected group consisted of 36 patients, 19 females and 17 males, suffering from acute or chronic pancreatitis. Participation in our survey was anonymous and voluntary. The study population was divided into two groups: 12 patients with acute pancreatitis (5 males and 7 females) and 24 patients with chronic pancreatitis (12 males and 12 females). The median age of patients was 42.4 ±14 years (range: 20–76 years). The duration of illness among the study population was varied. The biggest group of 16 (44.4%) patients suffered from pancreatitis for more than 5 years, 8 (22.2%) patients had pancreatitis diagnosed 1–2 years previously, only 3 (8.3%) subjects suffered from pancreatitis for shorter than half a year. To analyse patients’ nutritional knowledge an authorial questionnaire was used that contained 17 closed questions and open-type questions were used three times.ResultsMost (97%) subjects with acute and chronic pancreatitis adhered to the diet to various degrees, but almost 50% of people up to 35 years of age declared that they didn't respect the diet, or that they followed only a few recommendations. Despite each interviewer admitted to follow dietary recommendations for pancreatitis, 11.1% of patients could not indicate any of them. Most patients knew that their diet should contain a low amount of fat (61% of patients). Awareness of alcohol abstinence was confirmed in 44.4% of subjects. One third (33.3%) of patients knew that fried food should be avoided. Only a few patients were aware that smoking, spicy food, and strong coffee should be excluded from their diet. About 80% of the surveyed patients evaluated their level of nutritional knowledge as good or very good. Only 20% of the study population admitted their dietary knowledge was unsatisfactory, and most of these were woman (86%).ConclusionsThe results of the present study demonstrate that dietary knowledge among patients suffering from acute and chronic pancreatitis is insufficient. Although patients have theoretical knowledge about nutritional management, they have problems implementing the diet on a daily basis. It is necessary to pay more attention to nutritional education for patients suffering from pancreatitis.
