The Effect of Limited Perioperative Nonsteroidal Anti-inflammatory Drugs on Patients Undergoing Anterior Cruciate Ligament Reconstruction

Abstract: Administration of NSAIDs to patients after ACLR does not have a negative effect on graft survival, risk of revision, or risk of a KOOS-QOL score <44 at 2-year follow-up. We emphasize using caution when administering NSAIDs by keeping the duration and dosage of NSAIDs as short and low as possible to ensure sufficient pain relief while limiting unwanted exposure to any known and unknown adverse effects of these drugs.

“…The risk of revision was not influenced by the administration of non-steroidal anti-inflammatory drugs (NSAIDs). However, a subgroup of patients undergoing ACL reconstruction with patella tendon autograft showed a decreased risk of revision when NSAIDs were administered (HR 0.3 [95% CI 0.1 to 0.8], p=0.016) 18. In addition, the side of primary injury (right or left) was not associated with additional ACL reconstruction,15 and smoking or smokeless tobacco did not influence the 2-year risk of revision ACL reconstruction 12…”

“…34 However, commonly used anesthetics and NSAIDs such as bupivacaine and ketorolac tromethamine that reduce pain from tendinopathy are believed to exhibit cytotoxicity toward tenocytes and affect tendon healing. 35,36 Therefore, there is extensive concern regarding this therapeutic modality and a need to achieve balance between pain reduction and decreased cytotoxicity. Ascorbic acid has been shown to improve the rate and quality of rat Achilles tendon repair.…”

“…It is well established that NSAIDs is applied in modern multimodal pain therapy [ 26 ]. In addition, NSAIDs are commonly reported to prevent those common adverse side effects of opioids, including sweating, dizziness, nausea, vomiting, loss of appetite and dysuria [ 27 ].…”
