Background Patients with myocardial infarction (MI) undergoing percutaneous coronary intervention (PCI) experience a great deal of pain and discomfort in the femoral region induced by femoral catheter removal, which can stimulate the sympathetic system and affect some vital signs. Objective The aim of this study was to determine the effect of ice bag application to femoral region on pain and vital signs in patients with acute MI undergoing PCI. Methods This is a double-blind randomized controlled trial study. The sample of this study consisted of 60 patients with acute MI undergoing PCI, including 30 patients per group, selected by convenience sampling method and assigned to intervention and control groups by block randomization method. The intervention included the use of an ice bag in the femoral catheter insertion site. Data collection tools included demographic information form, Numerical Rating Scale (NRS), and vital signs form. Femoral site pain and vital signs were measured in both groups before, during, and after femoral catheter removal. The data were entered into SPSS ver. 22. Using descriptive statistics, Chi-square and Mann Whitney test, data were analyzed. Results Two groups were matched in terms of all demographic and disease-related variables (p>0.05). Pain score was lower in intervention group than in control group during and after the intervention (p<0.0001). Also, systolic, diastolic blood pressure, and temperature scores of the patients in the intervention group were significantly lower than the control group during and after catheter removal (p<0.05). Respiratory rate of the intervention group was significantly lower than the control group (p = 0.005). No significant difference between the two groups in the mean of heart rate in all three time periods (p>0.05). Result and Discussion Knowledge of non-pharmacological interventions which are cost effective and easily available is essential for the clinical nurses to alleviate pain and promote comfort for the patients. Nursing education should offer short-term continuing nursing education program on integration of pharmacological and non-pharmacological interventions in reducing pain and decreasing the need for narcotic analgesics. Conclusion Application of the ice bag on the femoral region is effective in stabilizing the vital signs and decreasing the pain caused by femoral catheter removal in MI patients undergoing PCI. Therefore, considering its low cost and complications, topical application of ice bag, as a nursing intervention to control the pain and keep stable vital signs, is recommended.

Sindrom Koroner Akut (SKA) merupakan suatu masalah kardiovaskular yang utama karena menyebabkan angka perawatan rumah sakit dan angka kematian yang tinggi. Sebagian besar SKA adalah manifestasi akut dari plak ateroma pembuluh darah koroner yang koyak atau pecah. Terapi reperfusi (sebisa mungkin berupa Percutaneus Coronary Intervention primer) diindikasikan apabila terdapat bukti klinis maupun EKG adanya iskemia yang sedang berlangsung, bahkan bila gejala telah ada lebih dari 12 jam yang lalu atau jika nyeri dan perubahan EKG tampak tersendat. Komplikasi mikrovaskuler sering dialami pada pasien setelah tindakan PCI yaitu nyeri pada daerah punggung atau back pain. Beberapa tindakan non farmakologi antara lain pemberian ice bag dan pengaturan posisi.Studi ini bertujuan untuk mengetahui penerapan ice bag dan pengaturan posisi untuk mengurangi back pain pada pasien post PCI. Metode yang digunakan adalah literature review. Pencarian artikel didapatkan dari Pubmed, Science Direct, Google Scholar dalam rentang waktu 2014-2022 dengan kata kunci “Percutaneus Coronary Intervention and Nursing” or “Percutaneus Coronary Intervention and Intrevention Nursing” or “Percutaneus Coronary Intervention and cold pack” or or “Percutaneus Coronary Intervention and positioning” or “Percutaneus Coronary Intervention and back pain” or “Comfort in care nursing and PCI” or “Comfort theory Kolcaba” didapatkan 1129 artikel. Pencarian artikel disesuaikan dengan kriteria inklusi dan eksklusi selanjutnya dilakukan review dengan hasil terdapat 20 artikel yang masuk dalam analisa untuk dilakukan tinjauan literature. Artikel-artikel tersebut menjelaskan efektifitas pemberian kompres dingin : cold pack dan pengaturan posisi pada pasien post PCI dengan cara menghambat perjalanan syaraf berdiameter kecil dalam menghantarkan rangsang nyeri. Simpulan dari studi di atas adalah ice bag dan pengaturan posisi efektif untuk menurunkan nyeri punggung pada pasien post PCI.

Introduction: Complications in catheterization vascular access range from 1%-61%. One of interventions that can be taken to prevent hematoma and reduce pain in post PCI patients at off sheath is with cold compresses or ice bag. The fact shows that pain and hematoma in patient PCI in hospital just done by using sand bag therapy. There was a limited review about the use of ice bag to reduce pain in patients with PCI.Methods: Searched articles through Science Direct, PubMed, EBSCO and Google Scholar conducted using advanced searching with keywords including “ice bag”, “pain”, and “percutaneous coronary intervention”. Searching through Google Scholar using keywords involving “effectiveness of ice bag on pain in patient’s percutaneous coronary intervention (PCI)”.Results: There were three articles control trials and the results showed that implementation of cold compresses with ice bag for 20 minutes before and after catheter release in PCI measures was significant in reducing pain (P <0.05).Conclusion: Giving cold compresses can reduce pain because it can inhibit the fibber of small-diameter nerves in delivering pain stimuli.
