Aim Oral mucositis is a debilitating side‐effect of conventional cancer treatment, particularly of the head and neck region. This review aimed to evaluate existing evidence to identify optimal nutritional interventions for oral mucositis management in adult populations receiving treatment for cancer. Methods CINAHL, PubMed, Embase and Scopus were searched from database inception to July 2019, with each eligible article critically appraised for risk of bias and assigned a quality rating. Certainty of evidence was appraised using the Grading of Recommendations, Assessment, Development and Evaluation system. Results Twenty‐three articles were identified (total unique study participants n = 7605). Nine intervention areas were identified. Certainty of evidence was moderate for oral cryotherapy in patients with solid or haematological cancers receiving 5‐Fluorouracil or high‐dose Melphalan chemotherapy prior to haematopoietic stem cell transplantation; and low for zinc supplementation for patients with oral cancer undergoing radiotherapy or chemoradiotherapy. Moderate certainty of evidence exists to recommend against glutamine supplementation in all cancer patients. Conclusion Research must determine the safety and efficacy of identified interventions to guide clinical practice. Addressing limitations requires larger, higher‐quality trials to increase evidence certainty.

ObjectiveTo investigate the effect of pre‐rehabilitation interventions such as nutrition and exercise for patients with head and neck cancer (HNC).MethodsWeb of Science, PubMed, Scopus, Google Scholar, and Cochrane databases were searched up to December 2022. Quality of life, length of hospital stay, postoperative complications, change in body mass index or muscle mass, and functional assessments were the primary outcomes. PRISMA guidelines were adhered to, and the study was registered on PROSPERO. The Cochrane Collaboration tool and Newcastle Ottawa scale assessed the quality of included studies. Pooled data are presented as odds ratios (OR) and 95% confidence intervals (CI). Analysis was conducted using RevMan5.4.ResultsA total of 31 articles were included for quantitative analysis and 15 for qualitative synthesis. Nutrition alone resulted in significant weight retention (2.60; 2.32, 2.88, p < 0.00001), length of stay (−4.00; −6.87, −1.13), p = 0.0006) and complications (0.64; 0.49, 0.83, p = 0.0009). Nutrition and psychoeducation resulted in a significant reduction in mortality rate (0.70; 0.49, 1.00, p = 0.05 and 0.60; 0.48, 0.74, p < 0.00001), and exercise resulted in a significant reduction in dysphagia (0.55; 0.35, 0.87, p = 0.01). Exercise with nutrition resulted in significant improvements in weight loss, length of stay, complications, and dysphagia. Randomized controlled trials (RCTs) had a moderate risk of bias and cohort studies were of fair to good quality.ConclusionPrehabilitation programs based on exercise, nutrition, or psychoeducation demonstrated improved post‐interventional outcomes in HNC, such as quality of life, and mortality and morbidity. Studies with longer follow‐ups and larger sample sizes, and investigations comparing nutritional supplements with exercise programs are needed.

Low muscle mass is prevalent among patients with cancer and a predictor of adverse clinical outcomes. To counteract muscle loss, β-hydroxy β-methylbutyrate (HMB) supplementation has been proposed as a potential therapy for older adults and various diseases states. This systematic review aimed to investigate the effects and safety of HMB supplementation in relation to muscle mass and function and other clinical outcomes in patients with cancer. A systematic search of MEDLINE, CINAHL, Embase, Cochrane Central Register of Controlled Trials, Scopus, ProQuest, and grey literature for reports published from inception to December 2021 was conducted. Included studies provided supplements containing any dose of HMB to adult patients with active cancer. A synthesis without meta-analysis was conducted using a vote-counting approach based solely on the direction of the effect (i.e. regardless of statistical significance). Risk of bias was assessed for each outcome domain, and evidence from higher-quality studies (i.e. those with either low or moderate risk of bias) was examined. Safety was evaluated using both lower-quality and higher-quality studies. Fifteen studies were included, in which six were randomized controlled trials in patients with various cancer types and treatments. Studies prescribed HMB combined with amino acids (73.3%), HMB in oral nutritional supplements (20.0%), or both supplement types (6.7%); Ca-HMB doses of 3.0 g/day were provided in 80.0% of the studies. Four studies had high risk of bias across all outcome domains. Considering the higher-quality studies, evidence of a beneficial effect of HMB supplementation was found in four of four studies for muscle mass, two of two for muscle function, three of three for hospitalization, and five of seven for survival. In contrast, no beneficial effects of HMB on quality of life or body weight was found in two of four and three of five studies, respectively. A limited number of higher-quality studies evaluating the impact of HMB on cancer therapy-related toxicity, inflammation, and tumour response were observed. No serious adverse effects directly related to the nutrition intervention were reported. Although limited, current evidence suggests that HMB supplementation has a beneficial effect on muscle mass and function in patients with cancer. Well-designed trials are needed to further explore the clinical benefit of HMB supplementation in this patient population.
