Revitalization procedures have been extensively studied during the last decade and offers several advantages over root canal treatment, such as the recovery of the natural immune system. Mature teeth have a small apical foramen diameter (AFD), which could impair the ingrowth of tissue into the root canal. We analysed three methods for apical foramen enlargement by instrumentation in in situ human teeth and evaluated the damage over hard tissues produced by the techniques. Tooth length (TL), defined as the length from the most coronal part of the crown to the point at which the file abandons the root canal, was calculated. Forty-four in situ teeth were randomized: Group I: instrumentation 0.5 mm coronal to TL; Group II: at TL level; Group III: 0.5 mm beyond TL. Teeth were instrumented up to K-file #80. The mandibles were scanned in a micro-CT device before and after treatment. Group I: Only 20 % of teeth presented an enlarged AFD, with augmentation of 0.09 mm. No damage to hard tissues was observed. Group II: 71.4 % of the teeth presented an enlarged AFD with augmentation of 0.42 mm. 35.7 % presented damage to periapical tissues. Group III: 86.7 % presented an enlarged AFD with augmentation of 0.43 mm. 46.7 % presented damage to periapical tissues. All groups presented similar prevalence of teeth with dentine thickness less than 1mm. All mandibular incisors presented areas of thickness less than 1mm. Instrumentation 0.5 mm beyond TL is the most effective technique.

To evaluate the postoperative pain at different times after endodontic treatment with and without the use of photodynamic therapy (PDT) of asymptomatic teeth with apical periodontitis. Material and Methods: In this cross-sectional randomized clinical trial, the treatment was performed in a single visit with and without the use of photodynamic therapy. The sample consisted of 10 patients with two teeth in the same jaw, but on different sides (splitmouth), subjected to the same endodontic treatment with reciprocating instrumentation in the apical foramen instrumentation limit. The teeth from each patient were randomly divided into two groups: G1 (Control): endodontic treatment without photodynamic therapy and G2 (PDT): received methylene blue 0.005% and irradiation with a low-level laser. The fillings were performed with a Touch'n Heat technical device and Ah Plus sealer. The treatments of G1 and G2 in the same patient were performed in an interval of 3 weeks between them and the blinding study was ensured during all long treatment. Postoperative pain was recorded using an adapted VAS scale in times of 6, 12, 24, 36, 48, and 72 hours. Results: In both groups, the highest value recorded at the VAS scale was 2 mm (no pain), and there were no statistically significant differences in postoperative pain between the groups at any observation times (p<0.05). Conclusion: The photodynamic therapy did not have advantages to control postoperative pain in endodontic treatments of asymptomatic teeth with apical periodontitis, since both groups showed low levels of pain in all patients evaluated.

The filling material should be restricted to the root canal, and not extend to the periradicular tissues. Overextension occurs when there is an overflow of gutta-percha and sealer, whereas overfilling refers to the overflow only of sealer beyond the apical foramen. Both may cause several negative clinical consequences. Nevertheless, an accurate diagnosis of where they occurred cannot always be performed by conventional radiographic examination, because of the two-dimensional aspect of the image. This paper describes a clinical case of labiomandibular paraesthesia after overfilling into the mandibular canal (MC), as diagnosed by cone-beam computed tomography (CBCT), later used to perform the treatment planning. A 34-year-old Caucasian female patient sought a private dental clinic complaining of pain in the right mandibular posterior region. After taking the anamnesis and performing clinical and radiographic exams, the patient was diagnosed with pulp necrosis in the second right mandibular molar, and underwent root canal treatment. The final radiography showed overextension or overfilling, probably into the MC. About 2 hours after the procedure, the patient reported paraesthesia of her lower right lip and chin. A CBCT confirmed a small overfilling into the MC. For this reason, vitamin B12 was prescribed as the first treatment option. After 7 days, the patient reported a significant decrease in paraesthesia, and was completely normal after 15 days. This case report shows that CBCT is an effective radiographic diagnostic tool that can be used as an alternative in clinical cases of labiomandibular paraesthesia caused by overextension or overfilling.
