The Effect of Endotracheal Tube Cuff Pressure Control on the Development of Microaspiration and Ventilator- Associated Pneumonia: Systematic Review

Abstract

xObjective: To investigate the effect of endotracheal tube cuff pressure control on microaspiration and ventilator-associated pneumonia. Materials and Methods: This study was created by using the Preferred Reporting Items for Systematic Review and Meta Analysis -Meta Analyzer-PRISMA checklist. Among the electronic databases, Medline/PUBMED, Scopus, Cochrane Library and Google Scholar were scanned for articles published before September 2018 and 1311 studies were reached. After reviewing the eligibility and exclusion criteria, seven experimental/quasi-experimental studies were included. The studies were evaluated with the Turkish Joanna Briggs Institute-Meta Analysis of Statistics Assessment and Review Instrument (JBI-MAStARI) Critical Appraisal Checklist in terms of methodological quality. Results: In one study, intermittent control method was used to avoid using any control method and intermittent control method was used despite pilot balloon finger palpation control method in another study. As a result of the methodological quality evaluations of the studies, the lowest score was five and the highest score was nine. It was determined that intermittent and continuous endotracheal tube cuff pressure control method was the most frequently used method in the ÖZ Amaç: Endotrakeal tüp kaf basıncı kontrolünün mikroaspirasyon ve ventilatör ilişkili pnömoni gelişimine etkisi araştıran çalışmaların incelenmesidir. Gereç ve Yöntem: Bu çalışma, Sistematik İnceleme ve Meta Analiz için Tercih Edilen Raporlama Maddeleri -Preferred Reporting Items for Systematic Reviews and Meta Analyses-PRISMA kontrol listesi kullanarak oluşturuldu. Elektronik veri tabanlarından, Medline/PUBMED, Scopus, Cochrane Library ve Google Akademik taranarak Eylül 2018 öncesi yayınlanmış çalışmalar tarandı. Arama sonucunda 1311 çalışmaya ulaşıldı. Uygunluk ve dışlanma kriterleri gözden geçirildikten sonra yedi deneysel/yarı deneysel çalışma dahil edildi. Çalışmalar, metodolojik kalite açısından Türkçe Joanna Briggs Enstitüsü MAStARI Deneysel ve Yarı Deneysel Araştırmalar Kontrol Listesi ile değerlendirildi. Bulgular: Bir çalışmada herhangi bir kontrol yöntemi kullanılmamasına karşı aralıklı kontrol yöntemi, bir başka çalışmada pilot balon parmak palpasyonu kontrol yöntemine karşın aralıklı kontrol yöntemi ve beş çalışmada ise aralıklı kontrol yöntemine karşın sürekli kontrol yöntemi karşılaştırıldı. Çalışmaların metodolojik kalite değerlendirmeleri sonucunda, en düşük puanın beş, en yüksek puanın da dokuz olduğu belirlendi. Çalışmalarda en sık olarak aralıklı ve sürekli endotrakeal tüp kaf basıncı kontrol yönteminin karşılaştırıldığı belirlendi. Mikroaspirasyon insidansı, üç çalışmada %18 ile %80 arasında, ventilatör ilişkili pnömoni insidansı ise, beş çalışmada %2,7 ile %67,7 arasında bulundu.
