The effect of domiciliary noninvasive ventilation on clinical outcomes in stable and recently hospitalized patients with COPD: a systematic review and meta-analysis

Abstract: IntroductionNoninvasive ventilation (NIV) improves survival among patients with hypercapnic respiratory failure in hospital, but evidence for its use in domiciliary settings is limited. A patient’s underlying risk of having an exacerbation may affect any potential benefit that can be gained from domiciliary NIV. This is the first comprehensive systematic review to stratify patients based on a proxy for exacerbation risk: patients in a stable state and those immediately post-exacerbation hospitalization. Methods… Show more

“…In line with the findings of the RESCUE trial, a recent meta-analysis of the effect of home NIV in COPD has reported no mortality benefit and no reduction in exacerbation frequency, with only a trend towards reduced length of stay and improved health-related QoL (38). This analysis does not take into account the results of the Chinese or UK trials and the results are unsurprising as the RESCUE study was the largest study in the meta-analysis recruiting more than 4 times the number of patients than any other included study.…”

“… 25 However, the effects of domiciliary NIV on clinical outcomes of chronic hypercapnic COPD patients have been contradictory in studies and meta-analyses. 26 28 While in one study, domiciliary NIV significantly decreased hospital admissions, 29 in another it demonstrated a borderline decrease, 30 and conversely in another study, it has been demonstrated that hospital admissions are less in patients receiving standard care compared to the patients using NIV. 31 In a multicenter Italian study, low-intensity NPPV with LTOT and LTOT alone were compared, and no difference was detected in terms of hospital admissions at the end of 2-year follow-up.…”

“…40 Recommendations for noninvasive ventilation use in patients with obstructive sleep apnea or obesity hypoventilation syndrome, although less controversial, also lack strong evidence in terms of long-term outcomes. [42][43][44] Monitoring and Follow-Up. Follow-up for patients is recommended with attention to addressing risk factors, symptoms, and exacerbations.…”
