The effect of desmopressin on nocturnal polyuria, overnight weight loss, and morning postural hypotension in patients with autonomic failure.

Abstract: We conclude that atrial natriuretic peptide can induce a gradual decrease in blood pressure, sometimes followed by symptomatic hypotension, which occurs suddenly. This phenomenon depends on the duration of the infusion as well as the dose. Moreover, the dose effect is enhanced during low sodium intake. This warrants careful observation of patients with essential hypertension who are given infusions of atrial natriuretic peptide over longer periods. The effect ofdesmopressin on nocturnal polyuria, overnight wei… Show more

“…58 The vasopressin analogue desmopressin, which acts on renal tubular vasopressin-2 receptors, reduces nocturnal polyuria and improves morning postural hypotension. 60 The peptide erythropoietin may be beneficial in some patients by raising red cell mass, secondarily improving cerebral oxygenation. 61,62 A broad range of drugs has been used in the treatment of postural hypotension.…”

“…801 DDAVP is a synthetic vasopressin analogue that acts on the V2 receptors of renal tubular cells to promote reabsorption of water and expansion of intravascular volume. It should be noted that DDAVP can induce a severe and lifethreatening hyponatremia, and that careful monitoring of serum sodium concentration is necessary during the first 4 to 5 days of treatment and at monthly intervals thereafter.…”

“…NP has been described but the times of overnight urine collection vary. Donaldson et al [12] arranged collections between 6 p.m. and 6 a.m., Wilcox et al [13] from 10 p.m. to 10 a.m. and Mathias et al [14] from bedtime to the time of getting up in the morning. For this study it was decided to standardise the collection period from midnight to 8 a.m. the next morning and NP was therefore defined as 'the production of 33% or more of the total 24-hour urine volume between midnight and 8 a.m.'.…”
