The Effect of Clomiphene Citrate on Different Sperm Parameters and Serum Hormone Levels in Preselected Infertile Men: A Controlled Double‐Blind Cross‐Over Study

Abstract: In order to try to clarify the often supposed stimulating effect of clomiphene citrate on spermatogenesis a controlled double-blind cross-over trial was used. 30 pre-selected normogonadotropic infertile men were given 50 mg clomiphene citrate daily or placebo in a double-blind cross-over fashion for three months, each treatment period being separated by three to four months. On clomiphene treatment the median sperm count increased from 13.3 to 28.7 mill/ml (P < 0.001). During the placebo treatment schedules th… Show more

“…This is probably due to a weak intrinsic oestrogenic activity of the compound which may explain some adverse effects observed during intake of high doses. Recently, however, Ronnberg (1980) was able to prove the efficacy of clomiphene in lower doses in a double blind study.…”

“…3,7 The efficacy of CC therapy on fertility is controversial, with studies showing improvement in semen parameters, but no statistically significant improvement in pregnancy rates. 8,9 For our patient, CC therapy resulted in increases in sperm count. In theory, this improved sperm count could have been due to the CC alone (since we don't have semen parameters prior to the initiation of the finasteride, the azoospermia could have been a pre-existing condition) or could have been a result of the CC therapy increasing the intratesticular androgen levels to compensate for the reduced "androgenicity" related to the finasteride use.…”

“…[13] Whereas, previous study recorded highly significant increase in FSH, LH and T levels during CC 50mg/day treatment in preselected infertile men for 3 months. [14] Also 10 oligozoospermic men treated with CC were showed significant increase in FSH, LH and T levels as an CC effect on sex hormone binding globulin in normospermic and oligozoospermic men study. [15] As treatment in hypogonadal infertile men by using CC to raising testosterone hormone recorded significantly increased in T level at 6 and 12 weeks of CC 25mg/day.…”
