The Effect of Alpha-1 Proteinase Inhibitor on Biomarkers of Elastin Degradation in Alpha-1 Antitrypsin Deficiency: An Analysis of the RAPID/RAPID Extension Trials

Abstract: The RAPID (NCT00261833; N=180) and RAPID Extension (NCT00670007; N=140) trials demonstrated significantly reduced lung density decline in patients with alpha-1 antitrypsin deficiency (AATD) receiving alpha-1 proteinase inhibitor (A1PI) versus placebo. Desmosine and isodesmosine (DES/IDES) are unique crosslinkers of mature elastin fibers and are utilized as measures of elastin degradation. The aim of this post-hoc study was to determine the effect of A1PI therapy on DES/IDES levels in patients from RAPID/RAPID … Show more

“…The RAPID trials support the efficacy of augmentation therapy in slowing disease progression during 4 years of treatment and, as lost lung density during placebo treatment never recovered following augmentation therapy in the delayed‐start group, the trials highlight the importance of early initiation of augmentation therapy (Figure ). In a post hoc analysis of the RAPID trials, treatment with AAT was also associated with reduced elastin degradation as evident by biomarkers isodesmosine and desmosine (Figure ) …”

“…Однако как и в предыдущих исследованиях в этой области, достоверного влияния лечения на другие конечные показатели, такие как легочная функция и КЖ, не выявлено [87]. В дополнительной информации об исследованиях также подробно описано снижение циркулирующего десмосина, что свидетельствует о положительном влиянии ЗТ на процесс разрушения эластина [109].…”
