The Effect of Acth on Plasma Testosterone and Androstenedione Concentrations in Patients With Prostatic Carcinoma

Abstract: The effect of Synacthen (β1–24-corticotrophin) on plasma testosterone and 4-androstene-3,17-dione concentrations in untreated patients with prostatic carcinoma, and in patients receiving endocrine therapy is described. An established specific radioimmunoassay was used for the measurement of testosterone, and a radioimmunoassay for 3-androstene-3,17-dione using thin layer chromatography has been developed. Administration of Synacthen resulted in a fall in testosterone in untreated patients, but a rise in 4-andr… Show more

“…The testosterone concentrations in the pe ripheral plasma of patients with carcinoma of the prostate reported here are in agreement with the data contained in the literature 0Gandy and Peterson, 1968;Sciarra et al, 1973;Cowley et al, 1976;Harper et al, 1976) and indicate that Leydig cell function in these patients is within the normal range of old men. All the patients with varicocele studied here and those reported on in the literature (Comhaire and Vermeulen, 1975;Swerdloff and Walsh, 1975) had normal plasma testoster one values.…”

“…So far, however, initial hormonal therapy in prostatic cancer has been only partial and limited to removal of the androgens of testicular origin. Since 5% to 10% of circulating androgens are of adrenal original, it is most likely that they continue to have a stimulatory effect on the growth of prostatic cancer after removal of testicular androgens [19][20][21][22].…”

“…'Residual' testosterone in the plasma of castrated pa tients, or those on DES or Zoladex, is of adrenal origin [7,23], although relapse or disease progression is rarely associated with a secondary rise in testosterone levels [24]. Adrenal Cm-steroids, dehydroepiandrosterone (DHA) sulphate, DHA and androstenedionc would seem to be metabolised peripherally to produce testoster one and are also utilised by the human prostate ( fig.…”
