The Effect of a Single Dose of Intravenous Ketamine on Suicidal Ideation: A Systematic Review and Individual Participant Data Meta-Analysis

Abstract: Ketamine rapidly reduced suicidal thoughts, within 1 day and for up to 1 week in depressed patients with suicidal ideation. Ketamine's effects on suicidal ideation were partially independent of its effects on mood, although subsequent trials in transdiagnostic samples are required to confirm that ketamine exerts a specific effect on suicidal ideation. Additional research on ketamine's long-term safety and its efficacy in reducing suicide risk is needed before clinical implementation.

“…Meta-analyses have confirmed and further supported the significance of both ketamine’s antidepressant [2528] and anti-suicidal [29] actions compared to placebo controls. This finding revolutionized and established the concept of rapid-acting antidepressant medications.…”

“…Relapse to depression following ketamine treatment might be related to the reversal of the synaptic effects of ketamine [39]; also see Sections 4 and 5. In addition to its antidepressant actions, a single infusion of ketamine induces anti-anhedonic effects [40] and reduces suicidal ideation [29] in depressed patients with an effect commencing within a few hours of administration and lasting for up to 1 week, similar to its antidepressant time course. Importantly, the anti-suicidal actions of ketamine are partially independent of its antidepressant effects [29, 41], although further studies are required to confirm this conclusion.…”

“…Included patients did not differ significantly in ketamine (pooled N = 93) vs control (pooled N = 74) groups on sociodemographic and important clinical parameters such as baseline severity of depression. The findings of this meta-analysis 15 are summarized in Table 1. In short, on almost all outcomes at almost all time points between posttreatment days 1 and 7, ketamine was superior to control intervention.…”

“…At least 2 RCTs, 18,19 not included in the meta-analyses discussed, 13,15 have recently been published. Both RCTs recruited patients with clinically significant suicidal ideation, operationalized as a Scale for Suicidal Ideation (SSI) score of at least 4.…”
