The Effect of a New Anular Repair After Discectomy in Intervertebral Disc Degeneration

Abstract: In this pilot study, the MPSS can provide effectively sealing for damaged anulus to withstand stresses. Direct repair of anular incision by this suturing method does significantly slow down degenerative process within discs after discectomy.

“…A major hindrance to the assessment of the efficacy of such therapeutic strategies has been the inadequacy of the animal models available to examine DDD (reviewed in [68]). Large animal models of DDD have been developed in dogs [76,77,119], sheep [70,71,72], goats [73,74,75] and pigs [79,80,81]. The latter two species suffer from anomalously high spontaneous healing responses and persistence of notochordal cells in the IVD with ageing, compounding unequivocal interpretation of the efficacy of therapeutic repair strategies.…”

“…Large animal models of DDD have been developed in sheep [70,71,72], goats [73,74,75], dogs [76,77,78], and pigs [79,80,81] and, of these, the sheep represents the gold standard large animal model. In 1990, Osti and colleagues developed a model of DDD induced by controlled anterolateral (5 × 5 mm) surgical defects over a 24 months post operative (PO) period [71].…”

“…Recently, several tissue-engineering studies have focused on the quality of repair tissue in comparison to native discs, particularly in view of replacement of the AF [5660]. Interestingly, however, there is still limited information on the effects of nerve ingrowth into the defect or the implanted site during IVD degeneration.…”

“…Additionally, the results of their previous work 38 were reconfirmed in that a slit-like incision best withstood internal pressurization forces, and by disrupting a smaller area of the outer annulus, the membrane has a higher potential to heal. In contrast, Chiang et al 44 introduced a modified purse-string suture technique, and published results showing ability of the repaired disc to withstand significantly higher pressures when compared to unrepaired annular defects and defects closed with a simple suture.…”
