ObjectiveNoisy galvanic vestibular stimulation (nGVS) is an effective method for stabilizing posture; however, little is known regarding the detailed muscle activity and joint movement in the standing posture. This study aimed to clarify the changes in the lower limb muscle activity and joint angular velocity by nGVS intervention using the simultaneous assessment method of inertial measurement units and surface electromyography (EMG).MethodsSeventeen healthy participants were assessed for their physical responses under four conditions (standing on a firm surface with eyes-open/eyes-closed, and a foam surface with eyes-open/eyes-closed) without stimulation (baseline) and with stimulation (sham or nGVS). Noise stimuli were applied for 30 s at a level below the perceptual threshold. The body control response was evaluated using EMG activity and angular velocity of the lower limbs.ResultRegarding the change from baseline for each parameter, there was a significant interactive effect of EMG activity in the muscle type × intervention and EMG activity and angular velocity in the condition × intervention. Post hoc analysis revealed that the angular velocity was significantly decreased in the abduction-adduction direction in the standing on a foam surface with eyes-closed condition compared to that with eyes-open in the nGVS intervention.ConclusionOur results suggest that nGVS altered physical responses in different standing postural conditions. The present study is exploratory and therefore the evidence should be investigated in future studies specifically target those muscle activities and joint motion parameters.

Introduction: Knee osteoarthritis (OA) is a chronic condition that results in pain, muscular fatigue, functional instability, and disability. Bracing and taping can be a suitable treatment for knee OA, hence the aim of this study was to compare the effects of a soft knee brace and kinesiotaping (KT) on gait and balance parameters in patients with knee OA. Material and methods: Twenty-three patients with knee OA, grade I or II were evaluated. Gait parameters were measured using the motion analysis system including gait velocity and stance time (of the affected limb). Balance parameters including Overall Stability Index (OSI) and Medial-Lateral Stability Index variables were assessed using the Biodex stability system. Patients were assessed in three situations: without a knee soft brace or KT, with a soft brace, and with KT. The time interval was one week between each measurement. A paired t-test was used to compare patients in the three conditions.Results: There were statistically significant differences in all gait and balance parameters except for the OSI after using kinesiotaping (p < 0.05) but not after bracing. There were clinical improvements in all outcomes after taping more significant than after a soft knee brace.Conclusions: Kinesiotaping is more effective than the soft knee brace for gait and balance improvement in patients with knee OA.

Fractures affecting the tibial plateau posterior aspect represent a difficult challenge in terms of treatment. Treating articular depression in tibial plateau fractures often involves elevating the fragment (s), residual defect filling utilizing bone grafts or substitutes, as well as providing support to the articular fracture reduction using screws via a plate positioned medially and/or laterally. Treatment of tibial plateau fracture may be non-operative or operative. Fracture proximal tibia can be surgically treated by various options including plate osteosynthesis, external fixator, (CRPF), (AAPPO) or primary total knee replacement. (ORIF) of tibial plateau fractures by plate as well as screws can be achieved by several types of plates and many approaches including plates [conventional non locked plates, locked plates, one third tubular plate], external spanning fixator and CRPF. There are some tibial plateau fractures complications including injury of vascular, nerve and ligaments, infection, nonunion, malunion, posttraumatic arthritis and stiffness.
