The Diagnostic Validity of Citric Acid‐Free, High Dose 13C‐Urea Breath Test After Helicobacter pylori Eradication in Korea

Abstract: The (13)C-UBT cutoff value currently used has poor specificity for confirming H. pylori status after eradication, and this lack of specificity is exacerbated in patients that have undergone multiple prior eradication therapies and in patients with moderate to severe gastric intestinal metaplasia. In addition, the citric-free (13)C-UBT would increase a false-positive (13)C-UBT result.

“…23 Fourth, we used a cut-off value of 2.5‰ for UBT to assess eradication success, but some of those diagnosed as eradication failures might have been false-positive, in such cases with intestinal metaplasia. 38 Fifth, we did not examine the rate of resistance to CAM, which is a key factor in eradication therapy. In conclusion, vonoprazan-based therapy was feasible and showed a high H. pylori eradication rate as a first-line regimen.…”

“…During the endoscopy, 2 biopsy specimens were taken from the antrum and corpus for rapid urease testing (Pronto Dry; Medical Instruments Corporation) and DPO‐based multiplex PCR point mutation testing. The 13 C‐urea breath test (UBT) cutoff value was set as 2.5‰, according to the manufacturer's recommendation (POCone ® ; Otsuka Pharmaceutical Co. Ltd); a delta 13 CO 2 of >2.5‰ was considered positive for H pylori infection . The presence of H pylori infection was defined as a positive rapid urease test and an additional positive result with 13 C‐UBT or anti‐ H pylori antibodies.…”

“…13 C-UBT cutoff value was set as 2.5‰, which is recommended by the manufacturer (UBit-IR 300 ® ; Otsuka Pharmaceutical Co. Ltd, Tokyo, Japan), and a delta 13 CO2 of >2.5‰ was considered positive for H. pylori infection. 19 The eligible subjects were randomly assigned to receive a 10-day concomitant therapy or a 14-day PAM-B therapy. The patient's hospital identification number (either even or odd) was used to allocate the subjects to the concomitant therapy or PAM-B therapy: if even, we prescribed the PAM-B regimen; if odd, the concomitant therapy was prescribed.…”
