GirişCREST sendromu, sklerodermanın bir formudur. Kutanöz kalsifikasyon, Raynaud fenomeni, özefageal dismotilite, sklerodaktili ve telenjiektazi olmak üzere 5 major semptomu mevcuttur. CREST sendromu tanısını koymak için bu 5 semptomdan en az üçünün saptanması gerekmektedir (1).Beş major semptomun bir arada bulunduğu CREST sendromu olgusu sunulmaktadır. Olgu SunumuKırk altı yaşında kadın hasta kliniğimize ayak tabanında sertlik, kızarma ve ağrı şikayeti ile başvurdu. Hastanın özgeçmişinde yaklaşık 20 senedir Raynaud fenomeni ve yaklaşık 5-6 senedir gastroözefageal reflü öyküsü mevcuttu. Dermatolojik muayenesinde sağ ayak beşinci metakarpofalengeal eklem üzerinde şişlik, kızarıklık, hiperkeratotik lezyon, sol ayak topuk ve plantar bölgede hiperkeratotik lezyonlar saptandı. Genel vücut muayenesinde mikrostomi, sklerotik yüz görünümü (Resim 1), yüzde ve elde telenjiektazi (Resim 1, 2), parmak uçlarında skleroz ve siyanoz (Resim 2), periungual eritem ve sol diz eklemi üzerinde fiks, sert, subkutan nodül (Resim 3) mevcuttu. Sistem sorgulamasında yutkunma güçlüğü olduğu öğrenildi. Hastaya bu bulgularla CREST sendromu tanısı kondu.Antisentromer antikor (ACA) değeri negatifti. CREST sendromunun primer bilyer siroz ile birlikteliğinin yüksek olmasından dolayı istenen anti düz kas antikoru (ASMA), antimitokondrial antikor (AMA) ve abdominal ultrasonografi sonuçları da normal olarak değerlendirildi.Özofagus tutulumuna yönelik çekilen özefagus pasaj grafisinde distal özefagus 1/3'lük kısımda yavaşlama, mukozal düzensizlik, tersiyer kontraksiyonlar saptanması üzerine yapılan endoskopisinde özefagusta polip ve postbulber duodenum sonrası şüp-heli obstrüksiyon saptandı. Endoskopi tekrarı planlandı fakat hasta tarafından kabul edilmedi. Transtorasik Dopplerekokardiyografi, elektrokardiyografi ve akciğer grafisi sonucu normal olarak değerlendirildi. Pulmoner hipertansiyon saptanmadı.Hastanın direkt grafileri değerlendirildiğinde sağ el 2. metakarpofalengeal ekleme yakın bölgede, sol el üçüncü parmak distal falanksın altında ve sol diz suprapatellar bölgede (Resim 4) kalsinozis kutis ile uyumlu bulgular saptandı.Göz konsültasyonu sonucu hastada kuru göz tespit edildi ve suni gözyaşı başlandı.Hastanın Raynaud fenomenine yönelik nifedipin 30 mg/gün, asetilsalisilik asit 100 mg/gün, özefagus dismotilitesine yönelik AbstractWe report a case of CREST syndrome (calsinosis cutis, Raynaud's phenomenon, oesophageal dysmotility, sclerodactyly and telangiectasia) with all of the five major symptoms. A 46-year-old woman was admitted to our clinic with the complaint of erythema, rigidity and pain on the plantar surface of the feet. She had had Raynaud's phenomenon for 20 years and oesophageal reflux for five years. Her face had become masklike and there was prominent telangiectasies on her face and hands. Sclerosis were confined to the fingers (sclerodactyly). Direct Xray graphy demonstrated calcinosis cutis on the left hand and suprapatellar region.
