“…A novel care pathway providing medicines optimisation pharmacist case management was piloted in the Western Health and Social Care Trust (Western HSCT) in NI in 2012-2014. [40][41][42] Within this care pathway intermediate care patients receive a continuum of pharmaceutical care throughout their stay delivered by a case management pharmacist who is an independent prescriber; a baseline medication review on admission informs the content of their personalised pharmaceutical care plan and directs the case management pharmacist on which clinical interventions to deliver. Case management then continues after the patient has been discharged from intermediate care, with additional clinical interventions delivered, if necessary.…”
