BackgroundImmunotherapy has become a pillar of advanced solid tumors treatment. Patients are more likely to benefit from neoadjuvant immunotherapy compared with traditional neoadjuvant therapy. However, the safety and efficacy of neoadjuvant immunotherapy for the treatment of locally advanced, surgically resectable Esophageal squamous cell carcinoma (ESCC) remain unknown.MethodESCC patients who received neoadjuvant treatment following minimally invasive esophagogastrostomy were enrolled from June 2020 to September 2021. The characteristics of neoadjuvant treatment and surgery were investigated to determine the safety and efficacy of the neoadjuvant combination of chemotherapy and immunotherapy (NCI).ResultsA total of 149 patients were included in the study. Patient ratio was 40:109 between NCI and neoadjuvant chemotherapy plus radiotherapy (NCR) groups. No significant difference was found in terms of pathological characteristics, including ypN stage, ypTNM stage, differentiation, lymphovascular invasion, perineural invasion, pathological complete regression and tumor regression score, and these parameters were not correlated with NCI or NCR (all p>0.05). Regarding to the operation, the NCI group had less blood loss (49.25 ± 13.47 vs. 57.02 ± 47.26, p<0.001), and shorter operation time (247.75 ± 28.28 vs. 285.83 ± 52.43, p<0.001) than the NCR group. Additionally, the NCI group demonstrated a lower rate of overall perioperative complications (p=0.003) and grade >2 perioperative complications (p=0.042) than the NCR group.ConclusionOverall, the findings reported here indicate NCI could result in better outcome and less complications to locally advanced ESCC patients compared with NCR therapy. As a novel therapeutic option, the efficacy and safety of NCI appears to be feasible and safe, while long-term survival data is still needed.

In this study, the combination of hyperspectral imaging (HSI) technology and band selection was coupled with color reproduction. The white-light images (WLIs) were simulated as narrow-band endoscopic images (NBIs). As a result, the blood vessel features in the endoscopic image became more noticeable, and the prediction performance was improved. In addition, a single-shot multi-box detector model for predicting the stage and location of esophageal cancer was developed to evaluate the results. A total of 1780 esophageal cancer images, including 845 WLIs and 935 NBIs, were used in this study. The images were divided into three stages based on the pathological features of esophageal cancer: normal, dysplasia, and squamous cell carcinoma. The results showed that the mean average precision (mAP) reached 80% in WLIs, 85% in NBIs, and 84% in HSI images. This study′s results showed that HSI has more spectral features than white-light imagery, and it improves accuracy by about 5% and matches the results of NBI predictions.

Background: Extensive research has indicated that tumor stemness promotes tumor progression. However, the underlying role of stemness-related genes (SRGs) in esophageal cancer (ESCA) remains unclear.Methods: This study identified differentially expressed stemness-related (DESR) messenger RNAs (mRNAs), microRNAs (miRNAs), and long non-coding RNAs (lncRNAs) in ESCA, and correlated them with the clinical features of patients with ESCA to develop a prognostic risk assessment model. Functional analysis, protein-protein interaction (PPI) analysis, competing endogenous RNA (ceRNA) networks, and tumor-infiltrating immune cell analyses were performed to corroborate the results obtained from the model.Results: Correlation analysis of the stemness enrichment scores revealed 1,106 DESR genes (DESRGs), 84 DESRmiRNAs, and 320 DESRlncRNAs were identified from The Cancer Genome Atlas Esophageal Carcinoma (TCGA-ESCA) dataset. Network clustering was performed and the top 20 connection points were identified, including CDC20 that connects to 136 adjacent nodes. A ceRNA network was constructed, including 17 DESRmiRNAs, 44 DESRlncRNAs, and 55 DESRGs.Conclusions: NCAPG [log 2 fold change (FC) =1.81; q value =2.68×10 −11 ] was significantly upregulated in ESCA and positively correlated with resting natural killer (NK) cells, suggesting that human NK cells rest via the overexpression of NCAPG in ESCA. hsa-miR-1269a is significantly upregulated in ESCA patients with poor prognostic features. CD4 + resting memory T cells (P<0.01) were significantly negatively correlated with hsa-miR-1269a. The insights presented in this study will contribute to the development of innovative therapeutics for the treatment of patients with ESCA.
