O objetivo deste estudo pautou em analisar o perfil epidemiológico de cárie dentária na Região Centro-Oeste do Estado de São Paulo, Brasil, em crianças de 12 anos de idade, segundo o gênero, comparando o índice CPOD em municípios fluoretados e não fluoretados. Foram utilizados os dados do Levantamento Epidemiológico do Estado de São Paulo - 1998, referentes à Direção Regional de Saúde - DIR X, representada por oito municípios. A amostra foi composta por 485 escolares, de ambos os gêneros, distribuídos pelos municípios. Os resultados demonstraram que na cidade de Pederneiras (médio porte, com flúor) o CPOD foi igual a 7,06, valor maior do que o encontrado no Brasil em 1986. A maior prevalência de cárie dentária ocorreu nos meninos, quando comparados às meninas, demonstrando uma nova tendência à cárie dentária na região centro-oeste. Não houve diferença estatisticamente significante entre CPOD de municípios de mesmo porte, independente da presença ou ausência de flúor na água, sugerindo o fenômeno da convergência. Possivelmente, pela ação da ingestão de outras fontes de flúor e a presença do efeito "halo". A prevalência de cárie na região esteve "alta", com CPOD igual a 4,82, não atingindo as metas para o ano 2000.

This paper presents a theoretical model to be used for comparative studies of lay and medically defined need for medical care. The model is based on an analysis of the concept of need and need statements, lay and medical concepts of ill health and the relationships between need, ill health and utilization of medical care. It is concluded, on both theoretical and empirical grounds, that need is relative to time, place and assessor. The most important determinants of assessments of for what and when one should seek medical attention are assumed to be the assessor's perception and valuation of the health status and his expectations of the probable out-come of medical care use. Assessment of need is seen as a process in which past experiences and expectations of the future course of the health status affect decision-making. It is emphasized that the points of view of both client and provider should be taken into consideration in medical practice, as well as in health care planning, because the two perspectives tend to be only partially congruent. The nature and extent of these incongruences could be studied by the application of the proposed model.

There are a lot of pitfalls threatening the quality management of our patients. Conflicts of interest, overuse, underuse and misuse of diagnostic and treatment modalities can harm patients and are difficult to detect. A lot of confounding factors are present. It is very difficult to define the right standard, because it will depend on your personal point of view and the quality of the evidence based system. Quality measurements must integrate the different elements of care, looking at health care as a complex adaptive system. We must be aware of budget restrictions, bureaucracy and last but not least: The Human Factor, Fraud and Money!
