The cut-off value of transient elastography to the value of hepatic venous pressure gradient in alcoholic cirrhosis

Abstract: Background/Aims: The hepatic venous pressure gradient (HVPG) reflects portal hypertension, but its measurement is invasive. Transient elastography (TE) is a noninvasive method for evaluating liver stiffness (LS). We investigated the correlation between the value of LS, LS to platelet ratio (LPR), LS-spleen diameter-to-platelet ratio score (LSPS) and HVPG according to the etiology of cirrhosis, especially focused on alcoholic cirrhosis. Methods: Between January 2008 and March 2017, 556 patients who underwent HVP… Show more

“…Secondly, HVPG, as a standard method to assess portal pressure, was not measured in this study [ 13 ]. Nevertheless, a reliable correlation between LS and HVPG has been widely reported in many studies [ 29 , 30 , 31 ]. Thirdly, LS-based, noninvasive models have inherent limitations for patients with severe ascites or obesity.…”

“…Although the evidence of a link between vascular fluid pressure and mechanical properties of the liver has been repeatedly reported [35][36][37][38][39][40] , the relationship between vascular flow and hepatic mechanical properties remains poorly described and very rarely mentioned 18 . In this work, an experimental protocol was proposed to investigate the latter relationship and preliminary results were reported in a limited number of subjects.…”

“…In addition, TE can predict not only the diagnosis of fibrosis, but also the occurrence and prognosis of various complications of cirrhosis. [18,19] However, liver stiffness measurement by TE requires additional expensive equipment other than the existing ultrasonic tool. In addition, it is difficult for the operator to directly identify the region of interest during the examination process, and thus the success rate decreases in patients with abdominal obesity, mass or ascites [20].…”
