The Current State of Mohs Surgery for the Treatment of Melanoma: A Nationwide Cross-Sectional Survey of Mohs Surgeons

Abstract: BACKGROUND The increased use of Mohs micrographic surgery (MMS) to treat melanoma has been accompanied by wide variations in practice patterns and a lack of best practice guidelines. OBJECTIVE The present study was a nationwide cross-sectional survey of Mohs surgeons to elucidate commonalities and variations in their use of MMS to treat melanoma. “…12 The histopathological assessment of melanocytic skin lesions is associated with considerable difficulty, particularly regarding MIS. [13][14][15][16][17] The intraobserver reproducibility from 187 pathologists who diagnosed MIS on H&E sections had only 60% agreement when the same slides were reviewed at least 8 months later. 14 The same study found only 40% interobserver agreement for the diagnosis of MIS when checked against a reference standard.…”

“…Per our institutional protocol [5], she was treated with of-label imiquimod prior to Mohs surgery, three times weekly for 4 months, without perceptible improvement, though given the lack of pigmentation in this neoplasm, it was noted that response was challenging to assess. She subsequently underwent slow Mohs micrographic surgery with immunologic staining for Melan-A and Sox10, without residual melanoma being identifed [5][6][7][8]. Te defect was repaired linearly and healed well.…”

“…13,22 A recent survey of Mohs surgeons and database analysis suggests wide-spread use of MART-1 in MMS for CM. 23,24 With techniques in MMS for treating melanocytic lesions evolving, there is an impetus to continue to evaluate emerging research for further clinical guideline development. To date, no systematic reviews have specifically examined the utilization of MART-1 staining in frozen sections for CMparticularly its effect on local recurrence.…”
