The concept of quality of life in dementia in the different stages of the disease

Abstract: In order to conceptually define quality of life (QOL) in dementia, the literature on QOL in the elderly population, in chronic disease and in dementia was studied. Dementia is a progressive, age-related, chronic condition and to avoid omissions within the dementia-specific concept of QOL, a broad orientation was the preferred approach in this literature study. Adaptation is a major outcome in studies investigating interventions aimed at improving QOL in chronic conditions, but to date, it has not been used in … Show more

“…The differences in the rating of QOL between self-report and proxy report generally depend on variables such as depressive symptoms and severity of dementia and should be recognized [55,61,62,63,64]. The QUALIDEM was developed to measure QOL in persons with dementia; however, observational instruments always have the possibility of observer bias [65]. The QUALIDEM was the last instrument filled by the nurse, and recall of earlier items could have influenced the answers.…”

“…In the early stages of dementia the positive ratings of patients can be understood in terms of psychological mechanisms, the disability paradox (adaptation to change) [32], or processes of "self-maintaining" and "self-adjusting" [33]. The more negative ratings of caregivers can be explained by the impact of the diagnosis and the functional and behavioral changes in the patient, which would produce greater burden.…”

“…Thus, there is a need for a tool to assess an individual's QoL as an important outcome measure both in clinical practice and in research. There is no universally accepted definition of dementia-specific QoL, but most specialists in the field agree that it is a multidimensional concept including the person's subjective experience of life and objective criteria related to activities valued by society [6,7,8]. …”
