The authors examined the stability of schizotypal (STPD), borderline (BPD), avoidant (AVPD) and obsessive-compulsive (OCPD) personality disorders (PDs) over 2 years of prospective multiwave follow-up. Six hundred thirty-three participants recruited at 4 collaborating sites who met criteria for 1 or more of the 4 PDs or for major depressive disorder (MDD) without PD were assessed with semistructured interviews at baseline, 6, 12, and 24 months. Lifetable survival analyses revealed that the PD groups had slower time to remission than the MDD group. Categorically, PD remission rates range from 50% (AVPD) to 61% (STPD) for dropping below diagnostic threshold on a blind 24-month reassessment but range from 23% (STPD) to 38% (OCPD) for a more stringent definition of improvement. Dimensionally, these findings suggest that PDs may be characterized by maladaptive trait constellations that are stable in their structure (individual differences) but can change in severity or expression over time.

The importance of measuring growth outcomes following a traumatic event has been highlighted in recent literature. Although reports of growth are abundant, the relationship between growth outcomes and post-trauma distress remains unclear, with studies yielding conflicting results regarding this relationship. The purpose of the present study was to explore the interrelationships among growth outcomes and measures of depression and posttraumatic stress disorder (PTSD) among 100 female treatment-seeking physical and sexual assault victims. Although the majority of women reported some degree of growth in this study, and growth scores were comparable to those from other samples, measures of depression and PTSD were not significantly related to growth scores. The implication of these findings and future direction for research are discussed. Despite this considerable body of research, the concept of posttraumatic growth, or the possibility that an individual can "grow" psychologically from an adverse life event, has gained increased attention in the past few years [5]. Although the term used to describe posttraumatic growth has varied across studies (e.g., "perceived benefits," "positive psychological changes," "stress-related growth") [6][7][8][9], the conceptual aim of measuring growth outcomes typically has been to assess whether individuals can in some way move beyond a premorbid level of functioning following a traumatic event [10]. Thus, within the posttraumatic growth framework, an adverse experience is viewed as a potential catalyst for positive psychological and interpersonal change.Studies examining a variety of adverse experiences such as disasters [11] Although reports of growth are common, the relationship between growth outcomes and psychological adjustment is unclear. It seems plausible that the ability to derive growth from a traumatic event could serve to ameliorate the negative psychological effects of the trauma.In support of this view, researchers examining a wide range of stressors have documented an inverse relationship between a number of growth domains and measures of adjustment [11,14,18,[21][22][23][24][25][26]. However, other studies have yielded mixed support for this relationship [7,13,27,28], while a third group of studies have failed to find a significant relationship between measures of adjustment and growth [15,17,29,30]. Individuals who experience repeated interpersonal trauma exposure often present with posttraumatic stress disorder (PTSD) with more complex features. There is currently no consensus regarding whether current evidence-based interventions for PTSD need to be tailored to better account for these complex features. However, one recommended adaptation is to adopt a phase-based or sequenced approach involving three phases, each with a distinct function. This paper describes the development of a 12-session Emotion- and Memory-Processing Group Programme, adapted from Cloitre’s Skills Training in Affective and Interpersonal Regulation (STAIR) phase-based treatment protocol. A single case series provided a preliminary examination of the group-based intervention’s efficacy for three groups of women with a history of repeated interpersonal trauma and PTSD with complex features (N = 15; age 19–46 years) at The Haven Sexual Assault Referral Centre in London. Results revealed significant reductions in: PTSD, complex features of PTSD, and depression, along with improvements in process measures of maladaptive cognitions and emotion processing. Results from this case series demonstrate that an Emotion- and Memory-Processing Group Programme holds promise for treating individuals with a history of interpersonal trauma in outpatient settings, and provides evidence to warrant the completion of a feasibility trial.
