oscopy, false positive results are not uncommon with double contrast radiology, and up to 29% of polyps may be missed, although this figure falls to under 10% for polyps over 1 cm in diameter.'3 The presence of diverticular disease makes the diagnosis of associated lesions more difficult,'0 14 some authorities recommending that colonoscopy should be performed in all patients with symptomatic diverticular disease. 14 Another important group comprises patients with overt or occult rectal bleeding with no cause found on rigid sigmoidoscopy or barium examination: lesions may be shown in 27-81% of such cases, including tumours in 3-1 1%.4 15 Accurate cost comparisons in our hospitals are difficult to make, but the greater necessity for colonoscopy after an initial barium study would suggest that a policy of performing endoscopy initially is likely to work out less costly. For this reason and for reasons of accuracy and expediency and to avoid multiple investigations total colonoscopy may be the preferred initial investigation for those with suspected colonic disease. We have shown that radiology is not a prerequisite for adequate colonoscopic examination. Role of dietary restriction in Haemoccult® screening for colorectal cancer

The aim of this study was to investigate the effect of dietary restrictions on the positivity rate and yield of neoplasia during a study based on Haemoccult screening for colorectal neoplasia. Of 18,925 participants who completed Haemoccult tests on a normal diet, 647 (3.4 per cent) were positive. All repeated the test with dietary restrictions: 251 remained positive and underwent further investigations which revealed 35 carcinomas and 169 adenomas in 129 subjects. The remaining 396 were sent a further Haemoccult test at 3 months. Of these, 317 (80.1 per cent) completed the test: 31 (9.8 per cent) were positive. Further investigation of the 31 positive subjects revealed four carcinomas and 20 adenomas. Retesting with dietary restriction will reduce the false positive rate of Haemoccult when used for colonic cancer screening. However, if the retest is negative a further test should be performed. To minimize the adverse effect this may have on compliance, we have allowed an interval of 3 months to elapse before this retest is requested.

Forty-six patients were diagnosed as having schistosomal, tuberculous, or amebic colitis over a two-year period using colonoscopy and biopsy. Both schistosomal and tuberculous colitis could be diagnosed by characteristic endoscopic and histologic features in the majority of cases. Colonoscopy provided the added advantage of endoscopic polypectomy at the diagnostic session itself. The yield of granulomas in tuberculous lesions was 100 percent, although acid-fast bacilli could not be recovered from any. The endoscopic picture of amebic colitis often resembles that of inflammatory bowel disease; hence endoscopic biopsies are of paramount importance in establishing a correct diagnosis, especially in developing countries where both diseases exist with considerable frequency.
