The Clinical Value of New Scoring System of Cervical Lymph Node

Abstract: To distinguish malignant cervical lymphadenopathy, we established a new scoring system based on ultrasound features. Two hundred sixty-three patients with cervical lymphadenopathy received ultrasonic examination and underwent ultrasound-guided core needle biopsy or fine needle aspiration. The scoring system was proposed by multivariate logistic regression analysis and compared with Liao scoring system (0.06 × age + 4.76 × shortest-to-longest axis ratio + 2.15 × internal echo + 1.80 × vascular pattern). A new s… Show more

“…Ultrasonography-based applications in many ultrasound techniques have been used to distinguish the cause of lymphadenomegaly or to differentiate them from neoplastic processes, such as elastography (7) and microvascular imaging (6) as well as scoring systems (8,9). In a study comparing the ultrasound, roentgenogram and CT findings of mediastinum in children with tuberculous infections, there were cases of mediastinal lymphadenopathy on ultrasound evaluation in most of the patients with normal radiographic evaluation, some of them also being confirmed with CT (10).…”

“…The inclusion criteria were as follows (meeting all 3 of the following items): (1) US examination revealed cervical lymph nodes with suspicious malignant features ( 18 , 19 ) such as hyperechoic, noncircumscribed margin, absence of hilum, gross necrosis, calcification, peripheral or mixed vascularity and shortest-to-longest axis ratio (S/L ratio) ≤0.5; (2) cervical lymph nodes were adjacent to important structures such as blood vessels and nerves; and (3) the maximum diameter of the cervical lymph nodes was ≤1.5 cm.…”

“…4,5 A previous study proposed a novel predictive scoring system based on US features that could identify malignant lymph nodes. 6 This scoring system could minimize errors caused by the operator's subjective judgment, thereby greatly enhancing the predictive accuracy of benign or malignant cervical lymph nodes. However, differentiation between metastatic lymph nodes and lymphomas is still rudimentary.…”
