The Clinical Spectrum of Hepatitis C Virus in HIV Coinfection

Abstract: The biochemical, virologic, and histologic spectrum of hepatitis C virus (HCV) in 66 consecutive patients with HIV-HCV coinfection and 119 HCV controls was compared: 86% of coinfected patients had CD4 counts >200 cells/mm3, 51% had a normal alanine aminotransferase (ALT) value, the mean HCV RNA titer was 5.7 log IU/mL, 92% of coinfected patients were of genotype 1, and the mean histologic activity index was 6.86 with advanced fibrosis in 32% of patients. The biochemical, virologic, and histologic findings of H… Show more

“…7,47,48 Few studies have addressed noninvasive markers of hepatic fibrosis in HIV/HCV coinfected patients. 37 Myers et al 36 studied the use of an index incorporating age, sex, ␣2 macroglobulin, apolipoprotein A1, haptoglobin, bilirubin, and ␥-glutamyltranspeptidase in 130 coinfected patients from a single French center.…”

“…However, diagnosis of cirrhosis in these patients can be made clinically and often does not require liver biopsy. We also recognize that a significant proportion of coinfected patients may have normal ALT levels, 47 which was an exclusion criterion for entry into APRICOT. Therefore, our model may not apply to these populations.…”

“…16,18,19 Although there could be several explanations for this result, the lack of selection bias in studying patients in an HIV clinic (rather than a liver clinic) probably contributes, because many of these individuals had not been referred for management of hepatitis C. However, the remarkable range of severity of liver fibrosis among the patients is also notable. Although minimal liver disease was detected in a significant proportion of patients, more than 25% already had advanced disease with histological evidence of bridging fibrosis or cirrhosis.…”

“…Treatment of both HIV and HCV is more complex in co-infected patients because of liver toxicity and drug interactions.
