The clinical significance of equivocal findings on spinal MRI in children with medulloblastoma

Abstract: A higher proportion of equivocal findings was identified in the SHH subgroup. This institutional retrospective review demonstrates equivocal findings are common, not associated with decreased OS and should not prompt increased dose of CSI.

“…In this issue of Pediatric Blood & Cancer , Bennett et al. report on the significance of equivocal spine magnetic resonance imaging (MRI) findings in children with medulloblastoma at the time of diagnosis . Cauda equina nerve root enhancement, nerve root clumping, and linear vascular enhancement are occasionally described in radiology reports and may generate uncertainty regarding clinical management and risk stratification.…”

“…Metastasis of medulloblastoma occurs in one-third of patients at diagnosis and in two-thirds at the time of relapse. Furthermore, the presence of metastasis is the strongest predictor of mortality among MB patients [89]. MS-based proteomics has been used to investigate the metastasis of MB [89].…”

“…These features are commonly encountered in SHH medulloblastomas and represent the sequelae of posttreatment inflammatory changes rather than recurrence or metastasis. Awareness of these equivocal findings on serial images is essential to obviate the need for more aggressive therapy (139,140).…”
