The Clinical Role of Cytochrome P450 Genotypes in Helicobacter Pylori Management

Abstract: This first pharmacogenomics study on the influence of different CYP genotypes on H. pylori therapy suggests that, as in Asian populations, CYP2C19 genotype patterns are probably also relevant in Caucasians receiving H. pylori eradication regimens that include omeprazole. The possibility of a favorable drug interaction mediated by CYP2C19 and CYP3A4 requires investigation.

“…Some studies on Kawabata et al, 2003), whereas others show lower cure rates in homozygous EMs (ϳ60 versus Ͼ90% in the other groups) (Furuta et al, 2001a;Inaba et al, 2002). Almost all of the studies of H. pylori eradication have been undertaken in Japanese patients, although available data in Caucasians suggest similar findings (Sapone et al, 2003;Schwab et al, 2004;Gawronska-Szklarz et al, 2005). It seems that genotyping could provide a useful aid to prescribing for H. pylori eradication, and initial data suggest that this effort may be cost-effective (Lehmann et al, 2003).…”

“…Overall, omeprazole and lansoprazole-based regimens produce lower eradication rates in homozygous EMs than in heterozygous EMs or PMs (Furuta et al, 2001b;Sapone et al, 2003). For example, a 7-day triple therapy regimen resulted in cure rates of 60% in homozygous EMs, compared with 84 and 100% of heterozygous EMs and PMs, respectively (Sapone et al, 2003).…”

“…CYP3A4 and IL1B -511 polymorphisms are also shown to have effects on eradication rates 45,46 . Moreover, absorption of orally dosed drugs is often influenced by an ATP-dependent efflux transporter, P-glycoprotein (P-gp).…”

“…Based on the generally accepted idea that therapeutic efficiency of omeprazole is influenced by the CYP2C19 genotype, some authors have suggested that this also affects the effectiveness of different PPIs. However, the less clear influence of the CYP2C19 genotype over further PPIs, support that this relationship actually depends on the kind of PPI (Chang et al, 1995;Sapone et al, 2003).…”
