RESUMENPresentamos un caso de liquen simple crónico vulvar izquierdo, en una paciente con diagnóstico psiquiátrico de trastorno bipolar, tratado repetidamente como micosis. Se realizó biopsia que confirmó el diagnóstico y se trató exitosamente con antihistamínicos orales y corticoides tópicos.

INTRODUCCIÓNSe conoce como liquen simple crónico (LSC) o liquen de Vidal, a la alteración crónica pruginosa localizada, que se caracteriza por una placa circunscrita bien demarcada con engrosamiento y liquenificación. La piel se engruesa, hiperpigmenta y liquenifica. Las áreas habituales son la nuca, caras de extensión de los antebrazos y de las piernas, escroto y vulva. También es conocida como neurodermatitis y se relaciona con el roce y los rasguños (1).
CASO CLÍNICOPaciente de 25 años, nulípara, sin anticoncepción, portadora de un trastorno bipolar en tratamiento y control. Refiere prurito vulvar izquierdo de inicio lento y progresivo asociado a descamación y acartonamiento de la piel de esta zona, de intensidad creciente, hasta aparecer ulceraciones lineales, secundarias a gratage nocturno inconsciente. Relata múltiples consultas y tratamientos tópicos sin resultado. Al examen se observa engrosamiento, inflamación, hiperpigmentación y áreas escamosas, con erosiones lineales que comprometen la zona izquierda. A derecha no se observan lesiones (Figura 1).Debido a la cronicidad de su patología y con una nula respuesta a tratamientos tópicos previos, entre los que se contaban antimicóticos puros o asociados a corticoterapia, se decide realizar una biopsia bajo anestesia local. Este procedimiento se realiza tomando una muestra de 5 por 5 mm en el centro de la lesión, incluyendo epidermis, dermis y subdermis.El resultado histopatológico fue informado como: muestra de piel revestida por epidermis con discreta papilomatosis irregular y presencia de aisladas bandas de fibrosis alrededor de los clavos
