The Choice of a Suture to Close Abdominal Incisions

Abstract: Burst abdomen, incisional hernia and sinus formation continue to bedevil the surgeon. Significant associated factors include postoperative wound infection and the suture material used. A series of experiments was therefore designed to test suture materials for their use in infected abdominal wound closure. The nearest to the ideal, is a monofilament non-absorbable suture (monofilament nylon). It has a low infectivity, resulted in satisfactory wound tissue strength when used in infected wounds, and retained its… Show more

“…This relationship is frequently not addressed in the relative comparison between different suture types in vivo. Several studies have shown black braided silk to elicit a greater FBR when compared to other commonly used sutures [43][44][45]. However, a single fiber of native silk is composed of several individual fibroin filaments and a single black braided suture comprises several extracted silk fibers.…”

“…A curiosity about the study was the choice to close all the wounds of all study groups with silk suture, potentially sensitizing the animals to the silk suture under investigation. Bucknall et al [45] examined four sutures in abdominal wound closure in a rat model 10, 30, and 70 days postimplantation. Histological examination revealed silk elicited a 'moderate' zone of inflammation compared to a 'minimal response' induced by multi-and monofilament nylon and multifilament PGA (Table 3).…”

“…We also observed areas of squamous metaplasia in the cervices of the Mersilene and Vicryl suture control animals, which was not seen with silk gel. It is known that Vicryl promotes a mild inflammatory response in the rat abdomen 35 and squamous metaplasia is a response to chronic inflammation. 36,37 We speculate that areas of squamous metaplasia may represent increased inflammatory responses to cervical sutures compared with the silk gel.…”

“…Vários trabalhos destacam que a sutura constitui um dos fatores essenciais para uma rápida e perfeita cicatrização de feridas 3,6,10,12,13 . Entretanto, não existe nenhum relato que descreva o papel da sutura em feridas irradiadas.…”
