The Changing Face of Survival in Rett Syndrome and MECP2-Related Disorders

Abstract: Purpose Survival in Rett syndrome (RTT) remains unclear. Although early estimates were grim, more recent data suggest that survival into adulthood is typical. We aimed to define survival in RTT more clearly and identify risk factors for early death. Methods Participants with clinical RTT or Methyl CpG Binding Protein 2 mutations without clinical RTT were recruited through the RTT Natural History study from 2006 to 2015. Clinical details were collected, and survival was determined using the Kaplan-Meier estim… Show more

“…137 This is considerably lower than the estimates of 50% at 50 years using the North American Database (data derived from 50% response to questionnaires administered to IRSA family members) 220 and 75% at 45 years using nine years follow-up of the US Natural History sample. 221 Both samples are large but select groups likely to be better resourced than the general US population.…”

“…All participants had MECP2 testing; participants with clinical Rett syndrome were included even if they lacked a mutation. Further details have been published previously, including age of symptom onset and diagnosis, and age and cause of death Tarquinio et al, 2012Tarquinio et al, , 2015a. History of seizure onset, frequency, and treatment of seizures, as well as associated features and comorbidities was recorded at the first encounter and updated at each subsequent visit.…”

“…In general, FMS-RS is a simple measure to use and provides a varied picture of the assistance needed to ambulate in different environments. Our knowledge about decline in walking in RTT needs to be increased as ambulation has a positive association with survival [9]. In RTT FMS-RS can provide detailed longitudinal information on daily ambulation which could be helpful in the understanding of associations to a possible decline in walking.…”

“…For most mutations, clinical severity increases with age [7]. Whilst mortality is increased, many live into adulthood and some into older age [8,9].…”
