The changing epidemiology of hepatocellular carcinoma in Asia versus United States and Europe

Abstract: Hepatocellular carcinoma (HCC) is a major disease worldwide. There were 782,000 estimated new cases globally in 2012 with the majority (76%) occurring in Asia, especially China. Although the etiologies are similar, the prevalence of each HCC risk factor varies in different geographic regions. In China, hepatitis B virus (HBV) infection is the major cause of HCC, whereas in Japan, United States (US) and Europe, hepatitis C virus (HCV) infection predominates. With the epidemic of obesity and diabetes, non-alcoho… Show more

“…Chronic hepatitis B virus (HBV) and hepatitis C virus (HCV) infections are still considered as the most common risk factors for HCC (60%) worldwide. As one of the HBV endemic areas, the incidence rates of HCC were quite high (13.4 per 100,000 in males and 4.0 per 100,000 in females) [ 22 ]. A recent retrospective study conducted in the biggest national tertiary referral hospital in Indonesia (the biggest country in Southeast Asia), still demonstrated poor survival of HCC patients.…”

“…2. "CEUS LI-RADS criteria were developed in the United States, where HCC histology, epidemiology, and imaging features are different from those in Europe and Asia (6)." We would like to clarify that CEUS LI-RADS was developed by an international working group of experts from North America and Europe with support of the American College of Radiology (ACR).…”

“…This finding may be associated with regional differences in the etiologies of HCC. Among the known risk factors for HCC, chronic hepatitis B viral infection, either with or without cirrhosis, is more commonly noted in Asian countries, while cirrhosis because of other causes (eg alcohol abuse and chronic hepatitis C viral infection) is the major cause of HCC in non‐Asian countries 22 . For CEUS evaluation, very coarse cirrhotic liver could be potential pitfall for CEUS evaluation, especially when the lesion is located in deep or subdiaphragmatic area 2 .…”
