The challenge of regular outcome assessment: Why do we fail?

Abstract: Is it practical or ethical to collect clinical follow-up data from psychotherapy patients? We describe our heavy data loss between 2001 and 2005 in an NHS specialist clinic and how we are trying to fix it.
