The challenge of perioperative pain management in opioid-tolerant patients

Abstract: The increasing number of opioid users among chronic pain patients, and opioid abusers among the general population, makes perioperative pain management challenging for health care professionals. Anesthesiologists, surgeons, and nurses should be familiar with some pharmacological phenomena which are typical of opioid users and abusers, such as tolerance, physical dependence, hyperalgesia, and addiction. Inadequate pain management is very common in these patients, due to common prejudices and fears. The target o… Show more

“…Methadone has improved safety and tolerability compared with morphine because of its lower affinity for the MOP receptor. Methadone is mainly used in patients with chronic cancer pain who have reached a high opioid dose or in patients with a history of substance abuse as part of a methadone maintenance program (MMT) [ 54 ]. In these patients using high-dose MMT, low serum testosterone levels have been reported: the percentage of hypogonadism in men reached 75%, while in women was only 21% [ 55 ].…”

“…Perioperative pain management in patients with chronic pain presents unique challenges, 71,72 particularly for patients with opioid tolerance or those vulnerable to opioid-associated risks. It can be more challenging to manage patients on long-term opioid therapy in the perioperative period compared with patients who are opioid naive.…”

“…However, the design of our study does not allow identifying specific reasons for the observed high opioid requirement in TdBUP patients, that is, whether this increased requirement can be attributed to pre-existing pain versus effects related to complex buprenorphine interplay at the level of opioid receptors. Regardless of the cause for the observed increased need for opioid, pain management for patients receiving TdBUP should be adapted from guidelines for treatment provided to opioid-tolerant patients 30. In our study, three patients receiving TdBUP had multiple procedures, and our analysis was unable to account for potentially correlated data for these patients because random effects are not estimable in such a scenario.…”
