The CHADS2 and CHA2DS2-VASc scores for predicting ischemic stroke among East Asian patients with atrial fibrillation: A systemic review and meta-analysis

Abstract: The CHA2DS2-VASc score is superior to the CHADS2 score in identifying 'low risk' East Asian AF patients. Rather than a categorical approach, Asian guidelines should adopt a 2 step approach, by initially identifying the truly low risk patients, following which effective stroke prevention can be offered to those with ≥ 1 additional stroke risk factors.

“…10,[26][27][28][29][30] Indeed, the CHA 2 DS 2 -VASc score has been shown to consistently better than the older CHADS 2 score in identifying low-risk patients, even among Asian patients. 31 Take our previous study for example, among 25 286 Asian AF patients with a CHADS 2 score of 0, the CHA 2 DS 2 -VASc scores of these subjects ranged from 0 to 3, and the annual risk of ischemic stroke can be as high as 4.47%. 4 However, the risk of ischemic stroke was still 1.15%/y for patients with a CHA 2 DS 2 -VASc score of 0, which is above the suggested threshold for the initiation of NOACs (0.9%/y).…”

“…AF patients other than those having “valvular AF” are known to have non-valvular AF (NVAF). As stroke risk among NVAF patients vary, different stroke risk stratification tools including the CHADS 2 score have been used over the years and currently the CHA 2 DS 2 -VASc score is recommended [ 9 , 22 , 23 ]. NVAF patients can be stratified into low, intermediate, and high risk to stroke depending on whether their CHA 2 DS 2 -VASc scores are 0, 1, or ≥ 2, respectively.…”

“…More than half of all cardioembolic strokes are related to AF . There is evidence indicating that the CHA 2 DS 2 ‐VASc score may be more clinically useful for predicting stroke in patients with AF than the CHADS 2 score …”
