SummaryWhat is known and objective: Migraine headache is a relatively common, debilitating condition that costs our healthcare system over 78 billion dollars per year. Riboflavin has been advocated as a safe, effective prophylactic therapy for the prevention of migraines. The purpose of this study was to provide a systematic review of the current role of riboflavin in the prophylaxis of migraine headache. Methods What is new and conclusion:Riboflavin is well tolerated, inexpensive and has demonstrated efficacy in the reduction of adult patient's migraine headache frequency.Additional data are needed, however, to resolve questions involving pharmacokinetic issues and pharmacogenomic implications of therapy. K E Y W O R D Smigraine, pharmacogenomics, pharmacokinetics, riboflavin | WHAT IS KNOWN AND OBJECTIVEMigraine headache is a common, often disabling neurologic condition characterized by painful headaches associated with nausea, vomiting, and hypersensitivity to visual, auditory, and olfactory stimuli. The worldwide prevalence of migraine headache is about 10%; however, it is slightly higher in the United States at 12%.1,2 Women experience migraine headache at a greater rate (15%-17%) as compared to men. 1,3Epidemiologic data suggest that while 38% of patients with migraine headache need prophylactic therapy, only 3%-13% currently take prophylactic treatment. 4 Prophylactic therapy should be considered in situations where the patient feels that the recurring migraine headaches interfere with their daily routines and responsibilities despite acute treatment. 1-4The goals of treatment for patients requiring prophylactic therapy are to decrease the frequency, duration and severity of attacks,

Aims We compared brain activation patterns between female multiple sclerosis (MS) patients with voiding dysfunction (VD) and those without. We aim to expand current knowledge of supraspinal correlates of voiding initiation within a cohort of female MS patients with and without VD. Materials and Methods Twenty‐eight ambulatory female MS patients with stable disease and lower urinary tract dysfunction were recruited for this study. Subjects were divided into group 1, without VD (n = 14), and group 2, with VD (n = 14), defined as postvoid residual urine of ≥40% of maximum cystometric capacity or need for self‐catheterization. We recorded brain activity via functional magnetic resonance imaging (fMRI) with simultaneous urodynamic testing. Average fMRI activation maps (the Student t test) were created for both groups, and areas of significant activation were identified (P < .05). A priori regions of interest (ROIs), identified by prior meta‐analysis to be involved in voiding, were selected. Results Group‐averaged blood‐oxygen level‐dependent (BOLD) activation maps demonstrated significant differences between groups 1 and 2 during initiation of voiding with group 2 showing significantly lower levels of activation in all ROIs except for the left cerebellum and right cingulate gyrus. Interestingly, group 2 displayed negative BOLD signals, while group 1 displayed positive signals in the right and left pontine micturition center, right periaqueductal gray, left thalamus, and left cingulate gyrus. The activation map of group 1 was similar to healthy controls. Conclusions Our results support the hypothesis that distinct supraspinal activation patterns exist between female MS patients with VD and those without.

Background and purpose Our objective was to study the association between the presence of a neurological disease and the comorbidity burden as well as healthcare utilization (HCU). Methods Using baseline data from the Canadian Longitudinal Study on Aging (CLSA), we examined the burden of five neurological conditions. The CLSA is a population‐based study of approximately 50 000 individuals, aged 45–85 years at baseline. We used multivariable Poisson regression to identify correlates of comorbidity burden and HCU. Results The lifetime prevalence of five neurological diseases is presented: epilepsy, Parkinson's disease/parkinsonism, stroke/transient ischaemic attack, multiple sclerosis and migraine. We found the somatic and psychiatric comorbidity burden to be higher in those individuals with a neurological disease (an 18–45% mean increase in the number of chronic conditions) as compared with the comparison group without a neurological disease, except for Parkinson's disease/parkinsonism. The presence of a neurological disease was associated with only a modest increase in the probability of visiting a general practitioner but was associated with a greatly increased probability of visiting a medical specialist (up to 68% more likely) or an emergency department (up to 79% more likely) and an overnight hospitalization (up to 108% more likely). Conclusions We found striking associations between our neurological diseases and increased comorbidity burdens and HCU. These findings are important for informing public policy planning as well as driving avenues for future research. The present study established the CLSA as an important research platform for the study of neurological conditions in an aging general population.
