The Brazilian experience of nucleic acid testing to detect human immunodeficiency virus, hepatitis C virus, and hepatitis B virus infections in blood donors

Abstract: The Brazilian NAT HIV, HCV, and HBV kit is an automated NAT system suitable for routine blood donor screening in a completely traceable process. The analytical sensitivity as well as the diagnostic sensitivity fulfilled all requirements set by the health ministry for blood donor screening. A significant number of transmission cases were prevented by the implementation of this important program.

“…Three seronegative HIV-viremic donations were detected. The NAT yield rate per million donations was 3.34 for HIV, which is slightly lower than the rates observed in Brazil, varying from 4.38 to 4.62 per 1,000,000 donations 5 . In the USA and Europe, these ratios are less than 0.5 and 0.66 per 1,000,000 donations, respectively, and the rates in Africa are highest (close to 36.78 per 1,000,000 donations) 6 .…”

“…The HIV/HCV/HBV/malaria NAT prototype (Bio-Manguinhos, Brazil) is a real-time nucleic acid amplification multiplex developed to be incorporated into the Brazilian NAT platform to detect HIV, HCV, and HBV [13]. In this new development, the Plasmodium sp.…”

“…The entire process from sample preparation to detection of amplification was monitored by the inclusion of an internal control (IC) in each individual test. The primers and probes target the C-terminal nucleotide sequence of integrase in HIV (VIC), the 5'UTR of HCV (FAM), and the S region of HBV (FAM) [13]. Plasmodium 18S rRNA gene was chosen as the malaria target.…”

“…Years ago, a network of laboratories performing HIV viral load determination was established in all Brazilian states, currently performing more than million tests per year (21). Moreover, since 2015 it is mandatory to supplement serologic screening of donated blood units with NAT for Hepatitis C Virus (HVC) and HIV, representing another 3-4 million tests per year performed in 14 blood centers located in different Brazilian regions (22). This previous and ongoing experience demonstrates that implementing HPV testing is feasible.…”
