The Bladder Microbiome, Metabolome, Cytokines, and Phenotypes in Patients with Systemic Lupus Erythematosus

Abstract: Contrary to dogma, the human urinary bladder possesses its own unique bacterial community with alteration of composition associated with disease states. Systemic lupus erythematosus (SLE) is a complex autoimmune disease often characterized by kidney impairment.

“…A recent study evaluated active SLE patients (score SLEDAI-2K = 16.16 ± 24.31 points) and reported a negative correlation between Megamonas abundance in the bladder microbiome and C3 complement levels. Notably, HCQ dosage did not affect microbial communities [31]. Conversely, gut Megamonas has been reported to be depleted in the gut of SLE patients and positively correlated with helper T cells 17 (Th17) [32].…”

“…These microbiomes are essential for immune homeostasis. Mounting evidence supports the notion that dysbiosis in the blood (5), gut (6)(7)(8)(9)(10)(11)(12)(13), oral cavity (10,(14)(15)(16)(17), skin (18), and bladder (19) is associated with SLE. It is worth noting that most previous studies have reported gut dysbiosis in SLE patients, but several have demonstrated that a balanced gut microbiome is accompanied by an imbalanced microbiome in other body niches, such as the blood (5) and oral cavity (16), when the same cohort of SLE patients and healthy controls (HC) were involved in the studies.…”

“…Specifically, the bacterial DNA isolation, 16 S rRNA sequencing, and bioinformatics ( Liu et al, 2023 ). The extraction of bacterial DNA from urine, high-throughput sequencing, and bioinformatics analysis have been comprehensively described in our previous research ( Liu et al, 2022 ). Here, we provide a brief overview: Following PCR amplification of bacterial DNA, extraction was carried out using AMpure XP magnetic beads (Beckman Coulter, Indianapolis, IN, USA).…”

“…When conducting a microbiome study, it is essential to account for potential sources of contamination and validate the accuracy of the sequencing method. In our study, we included negative control and positive samples to assess environmental DNA contamination ( Liu et al, 2022 ). Amplification was performed using primers 319F and 806R, targeting the highly sensitive V3–V4 region of the 16S rRNA gene.…”
