The Battlefield of Chemotherapy in Pediatric Cancers

Abstract: The survival rate for pediatric cancers has remarkably improved in recent years. Conventional chemotherapy plays a crucial role in treating pediatric cancers, especially in low- and middle-income countries where access to advanced treatments may be limited. The Food and Drug Administration (FDA) approved chemotherapy drugs that can be used in children have expanded, but patients still face numerous side effects from the treatment. In addition, multidrug resistance (MDR) continues to pose a major challenge in i… Show more

“…These regulatory mutations can enhance the efflux activity of these pumps, allowing the bacteria to expel multiple types of drugs and thereby develop resistance to various antibiotics. In cancer cells, MDR-related drug transporters, such as ABC transporters, are active in the efflux of drugs, conferring MDR to cancer cells ( Banerjee et al, 2023 ; Bo et al, 2023 ; Fan et al, 2023 ).…”

“… 1 Chemotherapy is pivotal in treating most pediatric cancers and contributes to improved clinical outcomes. 2 Children with hemato-oncological malignancies frequently undergo the insertion of tunneled central vascular access devices (CVADs) using tunneled external lines (TELs) such as Hickman central venous catheters (CVCs) to allow delivery of chemotherapy. In addition, Hickman CVCs are frequently used to infuse antibiotics or other medications and nutritional supplements into the pediatric population.…”

“…Other main adverse drug effects (ADR) of vincristine include alopecia, myelosuppression, and gastrointestinal toxicity [ 3 ]. Vincristine-induced peripheral neuropathy is the side effect experienced by almost all children who receive vincristine [ 4 , 5 , 6 , 7 , 8 ]. Those patients develop neuropathic pain such as tactile allodynia and hyperalgesia [ 9 ].…”
