BackgroundOver 90% of outpatient care in China was delivered at public hospitals, making outpatient experience in this setting an important aspect of quality of care.ObjectiveTo assess outpatient experience with different aspects of physician services at China’s public hospitals and its association with overcrowding of the hospital outpatient departments.Research designRetrospective analysis of a large survey of outpatient experience in Shanghai, China. We tested the hypotheses that patient experience was poorer with physician-patient communication, education, and shared decision-making and where and when there was greater overcrowding of the hospital outpatient departments. Ordered logistic models were estimated separately for general and specialty hospitals.Subjects7,147 outpatients at 40 public hospitals in Shanghai, China, in 2014.MeasuresPatient experience with physician services were self-reported based on 12 questions as part of a validated instrument. Indicators of overcrowding included time of visit (morning vs. afternoon, Monday vs. rest of the week) and hospital outpatient volume in the first half of 2014.ResultsOverall, patients reported very favorable experience with physician services. Two out of the 12 questions pertaining to both communication and shared decision-making consistently received lower ratings. Hospitals whose outpatient volumes were in the top two quartiles received lower patient ratings, but the relationship achieved statistical significance among specialty hospitals only.ConclusionsInadequate physician-patient communication and shared decision-making and hospital overcrowding compromise outpatient experience with physician services at Chinese public hospitals. Effective diversion of patients with chronic and less complex conditions to community health centers will be critical to alleviate the extreme workloads at hospitals with high patient volumes and, in turn, improve patient experience.

Despite the widespread pursuit of physician-health system integration, the evidence for factors affecting successful integration is uncertain and inconclusive. We sought to identify and categorize the organizational factors in the current landscape of physician-health system integration. We conducted a scoping review of the empirical literature on this topic, first surveying the theoretical perspectives that have been used in past studies in order to determine how theory has been used to explain and predict changing integration strategies over time. Second, we extracted factors that have been used to define the environment, physician group, hospital, care coordination, and health system success. From the 29 eligible articles, bargaining-market power theory and transaction cost theory were the predominant theories applied. We identified 48 organizational factors that comprise the landscape of physician-system integration. Our findings cumulated in a conceptual model that may help health care executives, policymakers, and researchers more effectively address the complexities of integration.

The aims of the study are to explore the satisfaction of health-care staff in Chinese public hospitals with different aspects of their organizational environment and to identify factors affecting this satisfaction. The satisfaction of hospital staff members with organizational environment could be associated with the quality of patient care and patients' satisfaction. The design of the study is in the form of a survey. A questionnaire survey was performed from April to November 2008 to collect demographic characteristics of hospital staff members and analyse which organizational environment factors (hospital security policy and professional care, environmental security, safety of operations and management of human resources) influence staff satisfaction. Hospital members' satisfaction scores were high for hospital security policy and professional care but lower for safety of operations, the security of the environment and management of the human resources (lowest). Multivariate analysis identified that hospital size (large hospitals scoring highest), department (non-clinical department such as administrative or logistics department), professional title (student), position (administration) and years of employment (<3 years) were independently positively associated with overall satisfaction with organizational environment (P < 0.05). These results indicated that in China, hospital staff members were mostly dissatisfied with the administration and management of human resources. The organizational environment of hospitals should be improved to improve staff satisfaction.
