The Association of Extent of Axillary Surgery and Survival in Women with N2–3 Invasive Breast Cancer

Abstract: Axillary lymphadenectomy was associated with improved survival in patients presenting with clinical N2-3 invasive breast cancer. Further studies, particularly in the neoadjuvant setting, are needed to identify breast cancer patients with advanced nodal disease who may safely avoid a lesser extent of axillary surgery.

“…When negative surgical margins can be achieved, breast conservative surgery is preferred over mastectomy [ 17 ]. Although the role of axillary dissection seems to diminish as standard procedure of breast cancer treatment and staging, it is indicated in cases with node-positive disease at diagnosis and axillary lymphadenectomy is associated with improved survival in patients presenting with clinical N2-3 invasive breast cancer [ 18 ].…”

“…They noted no statistical difference in OS between those receiving SLNB vs ALND 26 . On the other hand, Park et al evaluated women with cT1-3, cN2-3 disease (median age 56y) and noted an association between ALND and improved OS for patients with cN2 and cN3 disease 27 . This difference in the association between ALND and OS may be attributed to the size of the tumors in each study population: Bonneau et al examined patients with relatively small tumors (T1-2) compared to the study population of Park et al, which included T1-3 tumors.…”

“… 14 , 15 Axillary lymphadenectomy was associated with improved survival in patients presenting with clinical N2–3 invasive breast cancer. 16 Most surgeons perform axillary lymph node dissection in patients with positive axillary lymph nodes. There are many reasons for this, such as difficulties in treating axillary recurrence, the psychological impact of recurrence in patients, and problems in a rigorous follow-up.…”
