The association between positive outcome expectancies and avoidance in predicting the outcome of cognitive behavioural therapy for major depressive disorder

Abstract: These findings suggest that the impact that lower positive outcome expectancies have on therapy outcome can be attenuated if patients do not avoid dealing with emotionally difficult material in session.

“…The criteria also included, (f) some consideration of how long-term and enduring the difficulties have been, (g) the nature and degree of strategies which are used to avoid dealing with difficult problems, which were referred to as "security operations", which may inhibit appropriate exploration, and finally, (h) the ability of an individual to maintain a focus on their difficulties within therapy. There is evidence that scores on the SSCT predict treatment outcome (33)(34)(35)(36), with more recent factor analytic work using the SSCT suggesting that capacity to take part in CBT is related to treatment outcome (36).…”

“…In another recent study, Renaud, Russell, and Myhr () examined the associations between different suitability items in patients with anxiety and depression. They found that individual suitability items (i.e., security operations and optimism/pessimism for a positive outcome) interacted to predict better treatment outcomes in patients with depression and anxiety disorders.…”

“…There is some evidence that the presence of avoidance is a barrier to treatment success in adults with depression (Renaud, Russell, & Myhr, 2013); and in some cases early avoidant behaviour is predictive of a later diagnosis of PTSD in parents of children with cancer (Lindahl Norberg, Pöder, & Von Essen, 2011). Although these results have not been replicated in children, it is possible that the presence of high levels of avoidance may impede treatment efficacy.…”
