BackgroundPrevious studies suggested that elevated levels of plasma soluble triggering receptor expressed on myeloid cells 2 (sTREM2) was related to increased risk of death, cardiovascular events and cognitive impairment after stroke. We aimed to prospectively investigate the association between plasma sTREM2 levels and post-stroke depression (PSD).MethodsWe measured plasma sTREM2 levels in 590 ischemic stroke patients from the China Antihypertensive Trial in Acute Ischemic Stroke. The 24-item Hamilton Rating Scale for Depression was used to assess depression at 3 months after ischemic stroke onset, and PSD was defined as a score of ≥8. Logistic regression analysis was performed to evaluate the risk of PSD associated with plasma sTREM2 levels, and net reclassification index (NRI) and integrated discrimination improvement (IDI) were calculated to assess the predictive value of sTREM2.ResultsOf the 590 participants, 229 (38.8%) patients experienced PSD. The risk of PSD elevated significantly with plasma sTREM2 levels (Pfor trend=0.034). After adjusting for several covariates, the odds ratio for the highest quartile of sTREM2 compared with the lowest quartile was 2.41 (95% CI=1.35-4.31) for PSD. Multiple adjusted spline regression analysis further confirmed the linear dose-response relationship between sTREM2 levels and PSD (Pfor linearity=0.024). The addition of sTREM2 to a conventional model notably improved the risk prediction for PSD (category-free NRI=21.50%, 95% CI=5.92%-37.07%,P=0.011; IDI=1.43%, 95% CI=0.45%-2.42%,P=0.005).ConclusionsThe present study demonstrated that elevated plasma sTREM2 levels were associated with increased risk of PSD, suggesting that sTREM2 may be a promising prognostic biomarker for PSD.RegistrationURL:https://www.clinicaltrials.gov; Unique identifier:NCT01840072.

Neurological disorders are highly prevalent and often lead to chronic debilitating disease. Neuroinflammation is a major driver across the spectrum of disorders, and microglia are key mediators of this response, gaining wide acceptance as a druggable cell target. Moreover, clinical providers have limited ability to objectively quantify patient-specific changes in microglia status, which can be a predictor of illness and recovery. This necessitates the development of diagnostic biomarkers and imaging techniques to monitor microglia-mediated neuroinflammation in coordination with neurological outcomes. New insights into the polarization status of microglia have shed light on the regulation of disease progression and helped identify a modifiable target for therapeutics. Thus, the detection and monitoring of microglia activation through the inclusion of diagnostic biomarkers and imaging techniques will provide clinical tools to aid our understanding of the neurologic sequelae and improve long-term clinical care for patients. Recent achievements demonstrated by pre-clinical studies, using novel depletion and cell-targeted approaches as well as single-cell RNAseq, underscore the mechanistic players that coordinate microglial activation status and offer a future avenue for therapeutic intervention.

PurposeThis study aimed to translate the English version of the Short Orientation-Memory-Concentration (SOMC) test into a Chinese version, denoted the C-SOMC test, and to investigate the concurrent validity, sensitivity, and specificity of the C-SOMC test against a longer and widely used screening instrument in people with a first cerebral infarction.MethodsAn expert group translated the SOMC test into Chinese using a forward–backward procedure. Eighty-six participants (67 men and 19 women, mean age = 59.31 ± 11.57 years) with a first cerebral infarction were enrolled in this study. The validity of the C-SOMC test was determined using the Chinese version of Mini Mental State Examination (C-MMSE) as the comparator. Concurrent validity was determined using Spearman’s rank correlation coefficients. Univariate linear regression was used to analyze items’ abilities to predict the total score on the C-SOMC test and the C-MMSE score. The area under the receiver operating characteristic curve (AUC) was used to demonstrate the sensitivity and specificity of the C-SOMC test at various cut-off values distinguishing cognitive impairment from normal cognition.ResultsThe total score for the C-SOMC test and the score for item 1 on this test exhibited moderate-to-good correlations with the C-MMSE score, with respective ρ-values of 0.636 and 0.565 (P < 0.001). The scores for each of items 2, 4, 5, 6, and 7 yielded fair correlations with C-MMSE score, with ρ-value from 0.272 to 0.495 (P < 0.05). The total score on the C-SOMC test and the item score were good predictors (adjusted R2 = 0.049 to 0.615) of the C-MMSE score, and six items were good predictors (adjusted R2 = 0.134 to 0.795) of the total score. The AUC was 0.92 for the C-SOMC test. A cut-off of 17/18 on the C-SOMC test gave optimal performance: correct classification of 75% of participants, with 75% sensitivity and 87.9% specificity.ConclusionThe C-SOMC test demonstrated good concurrent validity, sensitivity and specificity in a sample of people with a first cerebral infarction, demonstrating that it could be used to screen for cognitive impairment in stroke patients.
