The association between implementation and outcome of a complex care program for frail elderly people

Abstract: A higher degree of implementation of the CareWell program did not lead to the prevention of functional decline in frail elderly people.

“…36 ICT-based practice models of integrated care were delivered through the formation of multidisciplinary teams consisting mainly of clinician, practice nurse, GP, primary care physician, community nurse and social worker, with additional studies recruiting physiotherapist (38.9%), occupational therapist (27.8%), mental health practitioner (19.4%), pharmacist (27.8%) and other health care professional.
