The Assignment of American Society of Anesthesiologists Physical Status Classification for Adult Polytrauma Patients: Results From a Survey and Future Considerations

Abstract: This study demonstrates fair interrater reliability beyond that expected by chance of the ASA PS scores among anesthesiologists and trauma surgeons when assessing adult polytrauma patients. Although the ASA PS is used in some trauma risk stratification models, discrepancies of ASA PS scores assigned to trauma cases exist. Future modifications of the ASA PS guidelines should aim to improve the interrater reliability of ASA PS scores in trauma patients. Further studies are warranted to determine the value of the… Show more

“…Not only is accuracy of the ASA-PS class necessary to ensure the appropriate preoperative workup, but consistent ASA-PS classification also ensures accuracy of survival prediction models as well as quality comparisons among institutions (Skaga et al 2007 ; Kuza et al 2017 ). The ASA-PS class is also used by NSQIP to compare quality of care among hospitals.…”

“…Perioperative variables were collected from the hospital's electronic records system; baseline characteristics included age, sex, body mass index (BMI, kg/m 2 ), albumin (ALB) level, previous abdominal surgery history, carbohydrate antigen 19-9 (CA 19-9) level, and American Society of Anesthesiologists Physical Status Classification (ASA score) [20]. We defined portal vein (PV)/superior mesenteric vein (SMV) abutment as PV/SMV compression or lumen narrowing.…”

“…Our results provide real-world evidence of differences in ASA scoring between surgeons and anesthesiologists after the 2014 ASA score modi cation, which have previously been studied only in hypothetical scenarios [18,24] or between anesthesiologists and Internal Medicine providers. [25] Furthermore, we found that discordant ASA scores are associated with adverse outcomes, particularly when the surgeonassigned ASA score is lower.…”
